My favorite, something like: "Just write a letter to the Judge. Once he hears about what happened he'll get the other side to acknowledge that they are wrong. It shouldn't take that long or cost a lot of money. There is no way they can defend themselves. They are clearly wrong. It's a slam dunk and we'll win this case." They also say the same general stuff about the demand letter, which gets thrown in the garbage usually.

Scammers are enrolling Medicare patients in hospice care for services they do not need or for services that are not provided. Enrollment can also occur without the patient's knowledge, allowing the scammers to bill Medicare for a patient that is not present in the hospice facility. Correct. With unearned/ fixed low income who can’t afford open market insurance, there is no debt consolidation offers. Only homelessness. By the time applications are complete the applicant is forced out of their home.

There are two primary types of health inspections, also known as surveys. The first is a standard recertification inspection, which CMS mandates the state must complete at a maximum interval of 15.9 months. While Salyersville Nursing and Rehabilitation Center had a survey in May 2025, the previous one was in April 2021, representing an illegal four-year gap. The second type is a complaint survey, triggered when residents or families report violations to CHFS. The state website shows zero complaint surveys were investigated at this facility during that entire four-year gap. Given the severe care deficiencies previously and later documented at Salyersville, the probability of actually having zero valid complaints during those four years is virtually non-existent.

This has been my passion for years and I have seen so much. The states running these investigations is the root cause. They’re often not done right, and as you said, there’s zero enforcement or accountability. State health departments lack real enforcement capabilities, and AGs don't even see, let alone act on, the reports. This is just one huge flaw in a completely broken system. There is no reason the states should be responsible for running the program operations, this needs to be moved federally to CMS.

Inspections mean little when facilities have a short time to say they improved but in fact don't improve. There are no checks/balances. Lawsuits go back several years and yet their 2025 survey showed several violations. The issue is punishment/enforcement. We have little.

In this country we allow the horrific deaths of our seniors in nursing homes & call it "skilled." I guess you can say it is because it takes the gov't, legislatures & an industry to make it all happen. Worse yet we allow the state sponsored killings. kentucky.com/news/politics-g…
