The challenge will be that even if AOT is reintroduced widely (which I hope it will be)…that it must be resourced to deliver for all who meet the criteria, rather than a caseload cap. The college have been unable to provide guidance on caseloads for cmht.

But then they'll bring in the s2/s3 amendments and there will be fewer admissions - all part of the master plan to detain fewer and fewer people. It's expensive you know - easier and cheaper to push costs to CJ system and just promise that lessons will be learned.

In fact, consultant psychiatrists have in reality also been displaced from leadership of community pathways. They are only nominally consultant led. Key pathway decisions no longer necessarily rest with consultants. Admitting rights removed. x.com/Megsenmumdr/status/205…

But for OP work we still need Consultant Psychiatrists (for now) It’s only when these patients are most sickest who have to be IP & sectioned under MHA & liberty deprived, then they don’t need Consultant led care, Brilliant RCs who have done robust portfolio can takeover 😬

GP referrals will now be ‘assessed’ by a ‘Consultant led’ Single point of access 1 in 4 is the target to reject GPs & patients must insist on Jess’s Rule in referrals Any rejected referral must have a Consultant name & GMC number This is the only way to ensure patient choice
Solution to this could be whenever a patient's referral gets rejected, write to hospital asking for name and GMC number of consultant that rejected it. Shift burden back into secondary care > becomes unworkable > drive reform back the other way.

Thora Hird's performance in Alan Bennett's Talking Heads is always singled out, and quite right too. But Patricia Routledge's performance as Irene Ruddock in A Lady of Letters breaks your heart. I've never forgotten this scene. Patricia was an incredible dramatic actress.
