#ASCO26 #TRITON @FSkoulidis I’m struggling to understand the value of highlighting secondary endpoints while the primary endpoint remains blinded. The reported improvements in ORR and DoR for tremelimumab durvalumab chemotherapy versus pembrolizumab chemotherapy are numerical rather than compelling, and small patient numbers. Without the 1ry endpoint, it is impossible to know whether these findings represent a meaningful clinical advance or simply noise. Let’s wait for the study to read out before drawing conclusions. #NSCLC #LCSM

Negative phase III trial of concurrent thoracic RT in ES-SCLC receiving chemoimmunotherapy. TRT was initiated early (between cycles 2 and 3), and failed to improve outcomes: • Median OS 10.0 vs 11.8 months • No improvement in response rate • More adverse events and treatment-related deaths While disappointing, these results should not be extrapolated to all consolidative TRT strategies. One key difference from SAKK 15/19 is timing: RT here was delivered concurrently with ongoing chemoimmunotherapy, whereas SAKK 15/19 administers TRT after completion of induction treatment and no major toxicity was detected Perhaps the lesson is not that thoracic RT has no role, but that timing matters. pubmed.ncbi.nlm.nih.gov/4134… #ASCO26 #SCLC #LungCancer #Radiotherapy

Absolutely. Randomized confirmation needed? Of course.Practice changing? Debatable. But if I see a stage III ALK patient tomorrow, these data would be very hard for me to ignore. One of the most awaited presentations( at least by me) #ASCO26: HARMONi-6 shows that ivonescimab chemo significantly improves OS Vs tislelizumab chemo in first-line advanced Sq #NSCLC (HR 0.66). A positive phase III study with a clinically meaningful OS benefit and simultaneous publication in @TheLancet: thelancet.com/journals/lance… Caveat: this was a study conducted entirely in China. How these data will translate globally remains an important question? Let’s see what HARMONI 3 will show

In the phase III #LIBRETTO432 trial, adjuvant #selpercatinib improved event-free survival in resected stage IB–IIIA #RET fusion-positive #NSCLC. HR 0.17, with only 4 events vs 19 in the placebo arm.Overall survival remains immature, but today it is difficult not to be impressed by these data. REVOL858R delivers a clear message: sequential EGFR inhibition with erlotinib ramucirumab followed by osimertinib did not improve the primary endpoint (TFS) compared with upfront osimertinib in EGFR L858R-mutated advanced NSCLC.
