So proud of @louisecollier30 presenting her experience of setting up the heart failure specialist nurse in-reach service at @BucksHealthcare at the Oxford Heart Failure Specialist Day. This has had such an impact on patient care- proactive identification of patients, double the number of patients receiving specialist reviews, a huge uptick in the number of patients on guideline directed medical therapy and a 30% reduction in re-hospitalisation 👏👏👏👏
