The ideal puncture site can be identified only by echo!!!!! I think that transesophageal or intracardiac #echofirst guidance should become the only way to perform transeptal puncture!
Where is the ideal site for transseptal puncture? In the 40° RAO projection, a pigtail catheter placed in the non-coronary aortic cusp helps define the posterior border of the aortic root. The optimal puncture site (X) is typically located 1–3 cm inferior to the posterior boundary of the aortic root, within the fossa ovalis region. Accurate localization is critical to safely access the left atrium while minimizing the risk of aortic puncture and other complications. Reference: Croft CH, Lipscomb K. Noninferiority margins need clinical justification, and intention-to-treat and per-protocol analyses should ideally point in the same direction. Today, on International Clinical Trials Day, We are proud to share that the Dig-RHD trial has been published in JAMA. This is the first randomized controlled trial to test a heart failure therapy specifically in patients with rheumatic heart disease a condition that affects more than 40 million people worldwide, overwhelmingly in low- and middle-income countries, yet has long lacked high-quality trial evidence. It was a privilege to contribute to this study as part of the cardiology team at Sri Ramachandra Institute of Higher Education and Research, working alongside investigators from across the country under the leadership of Dr. Ganesan Karthikeyan (AIIMS, New Delhi) @drkarthik2010 . Finally published: the COBRRA trial, the first randomized head-to-head comparison of major DOACs — something the companies would never have done themselves, as a direct confrontation goes beyond their commercial interests. This was driven by independent investigators. The results somewhat support the idea of apixaban being the “safer” DOAC. That said, the list of study limitations is long, and for me, labeling something as “safe” is not enough — after all, even placebo is “safe” when it comes to bleeding. nejm.org/doi/full/10.1056/NE…
