🫀 Pulmonary vein anatomy is more variable than many clinicians realize. Pulmonary veins are not only an anatomical structure—they represent the principal arrhythmogenic region in atrial fibrillation, harboring approximately 85% of AF triggers and 50% of the arrhythmogenic substrate. ⚡ 📊 Common anatomical variants include: 🔹 Four separate pulmonary veins (70%) 🔹 Left common pulmonary vein trunk (30%) 🔹 Right middle pulmonary vein (20%) 🔹 More than four pulmonary veins (10%) 🔹 Right common pulmonary vein trunk (5%) 🔹 Other variants (5%) Recognition of these anatomical patterns is essential for accurate imaging interpretation, procedural planning, and successful pulmonary vein isolation. 🎯 📖 Full AF guidelines (2026) 👉 AFguidelines.com #cardiology #medicaleducation #paramedic #MedstudentTwitter #CardioTwitter #FOAMed #MedEd #CardioEd #AFib #EPeeps

How can we better describe procedural complexity in left atrial appendage closure? This live case demonstrates best practices in conduction system pacing (CSP), led by Amr Abdin. Using Boston Scientific stylet-driven leads, the procedure focuses on precise septal orientation and signal mapping. The RAO and LAO fluoroscopic views, along with the TAVI valve as an anatomical landmark, guide positioning to ensure optimal lead placement around two centimetres below the valve. The workflow highlights careful rotation and the use of the locking helix tool, balancing efficiency with accurate electrical capture while avoiding unnecessary use of contrast or His mapping. #EPeeps #CSP #Electrophysiology #Cardiology

🔍 #EHRATopicWeek on Atrial Tachycardias Atrial Tachycardia is more complex than we once thought Traditionally, rapid regular atrial arrhythmias were divided into “atrial tachycardia” and “atrial flutter” based mainly on ECG appearance. Today, high-resolution mapping shows these distinctions are often oversimplified. 📌 Current concepts classify all regular atrial rhythms >100 bpm as AT: • Non-reentrant AT • Reentrant AT (localized or macro-reentrant) Importantly, many arrhythmias previously labeled as “flutter” may actually represent different AT mechanisms. ⚡ Differentiating AT from organized AF can be challenging, especially in scarred atria or after ablation. ECGs, EP studies, intracardiac mapping, and even wearable devices all play an important role in diagnosis. 💡 Precise classification matters — for diagnosis, treatment strategy, and improving ablation outcomes. #EHRATopicWeek on Atrial Tachycardias Catheter ablation has become the preferred treatment strategy for most symptomatic or recurrent ATs, while antiarrhythmic drug therapy often shows limited long-term efficacy. 📌 Key concepts from the latest evidence: • Beta-blockers and calcium channel blockers remain important for rate control • Catheter ablation is now first-line in many patients • High-resolution electroanatomical mapping has transformed our understanding of AT circuits • Entrainment and activation mapping remain essential tools for distinguishing focal vs reentrant mechanisms • Pulsed-field ablation and ultra-high-resolution mapping are opening a new era in AT treatment 💡 Success in AT ablation depends on identifying the “critical isthmus” — the key region maintaining the arrhythmia — while durable bidirectional block is increasingly recognized as a crucial procedural endpoint. Importantly, recurrence remains a major challenge, especially in patients with structural heart disease, prior AF ablation, or extensive atrial scarring Find out more in the #EHRA_ESC Consensus document on Atrial Tachycardias 👉bit.ly/4wk5sBn @escardio @EuropaceEiC

The 2026 AHA/ACC/ADA/ASN CKM Guideline recommends the use of the PREVENT equations to (1) define CKM syndrome stage 3, (2) decide when to test for subclinical cardiovascular disease; and (3) guide risk-based decision making for CKM therapies. This scientific statement serves as a companion to this guideline and details the rationale for the use of outcome-specific PREVENT equations, the evidence base for selected risk thresholds, and the potential population-level impact of these recommendations. ✍🏼 @HeartDocSadiya @RangaswJ @pnatarajanmd @dramitkhera @NicoleBhaveMD @yuan_lu1 @rblument1 @JenHoCariology @virani_md @ChiadiNdumele
