I completely agree. Why has the importance of design (of equipment, environments, units, processes) become lost in our pursuit of safety. I recall in the early days spending hours talking about this. It seems to have got lost.
The Patient Safety Incident Response Framework (PSIRF) is coming soon. To support providers to start the process of preparing for #PSIRF we will be hosting a webinar on 5 September.

Agree. For safety - NHS needs to focus on views of staff and service users, culture, accessible use of data to improve practice; education of staff throughout the organisation and on collective responsibility for patient safety and value and behaviour towards one another.
I am happy to be corrected but it saddens me that those specialising in safety seem to be spending all their time focusing on incident reporting and investigation when the science of safety is so much more than that.
What a treat to end the day - open conversations on safety with our @NHS_ELFT Chair and NEDs - three incredible women with huge amounts of expertise & great dedication to us prioritising this crucial agenda.
