We will present latest results of NHS study on genetics metal ion levels in painful knees @ personalized arthroplasty meeting next month, Chicago. We have 4 UK sites and 2 international actively recruiting, study in set up at 4 others. @markbowditch65 would yr unit be interested?

Some nice early results from our Arthrogenex study. Titanium concentrations in patients with nexgen LPS TKRs attending review clinics at 4 to 10 year follow up. Correlation coefficient-0.7, p=0.002
Funding in place for 1750 patients, outcome scores, blood metal ions and genetics.

We identified HLA allele which places pts at greater risk of painful inflamm responses to CoCr in joint replacements. Image below shows the freq of the allele in populations across the world.
Darker the colour=greater frequency, white no data.
Green = 10-20%

ALVAL is clearly a major contributor to unexplained pain.

#Unicompartmental knee analysis looking good now.
Key findings so far:
1. Volumetric poly wear of explanted unis is 1.5 > than in total #kneearthroplasty (why?!)
2. Backside surfaces show wear (red), rather than the deformation we tend to see with contemporary #TKR

Unexplained pain post #arthroplasty
Could it be due to adverse reaction to metal debris? This is determined by metal exposure and genetics.
New test available on NHS combines ICPMS metal levels genotyping.

Great example of material loss from tibial tray. The poly insert has even managed to imprint its catalogue number....
