Providers like Lovelace are explicitly citing the "challenging policy environment" created by Medicaid under-reimbursement, heavy mandates, regulations, & the uncertainty around single-payer/public-option pushes. That's not theory. That's what they said. Insurers have problems, sure. But pretending state policy plays zero role is either uninformed or dishonest. Single-payer would expand the broken low-reimbursement model statewide & accelerate provider flight, especially in rural areas. If you have data showing it's "100% insurance issues," bring it, I'd love to hear it! Otherwise, calling real engagement "bots" while refusing to engage with the substance doesn't help New Mexicans. We need price transparency, real competition, tort reform, & serious investment in training & keeping doctors & nurses here, not more policies that drive them out. Lovelace's own statement on the 43 cuts: rising costs, shifting payor dynamics, & a "challenging policy environment." That environment is created by state lawmakers with Medicaid rates that have long under-reimbursed providers (even after recent targeted bumps to 150% of Medicare for some primary/behavioral/maternal services), coverage mandates, heavy regulations, taxes, & the uncertainty from public-option/single-payer pushes. Providers like Lovelace & Presbyterian have said exactly this. The Lovelace/BCBS "fiasco" you keep citing? It was a contract negotiation that ended last month, resulting in a new 4-year in-network agreement. Patients stayed covered. That's normal business in a regulated market, not proof that insurers alone "f*cked up" the entire system. I'm not some outsider guessing. Over a decade inside New Mexico's healthcare systems at bedside, compliance, program coordination, leadership, health committees & more. I've seen the broken incentives firsthand. Insurers add real administrative burden & denial games, yes. But government policy sets the rules & the payment floors. Pretending Medicaid underpayment, mandates, & single-payer rhetoric play zero role is either ignorance or deliberate dishonesty. Single-payer (or the "work toward a public option" path @DebHaalandNM has signaled) would expand the low-reimbursement model statewide, impose price controls, & accelerate provider flight. New Mexico already has physician shortages, rural access problems, & hospitals at risk. More underpayment means more cuts, longer waits, & worse outcomes, especially in rural areas. New Mexico ranked 49th in a recent national child well-being report. That ranking is a reminder that we still have work to do. In recent years, New Mexico has improved graduation and literacy rates, reduced child poverty, increased family income, expanded childcare access and made historic investments in children and families. The work isn't finished, but we will continue investing in children, families and opportunities because every New Mexican deserves the chance to succeed.
