🚨Neuroendocrine marker expression in mucosal melanomas of the H&N might represent an important diagnostic pitfall!
Shah et al. in IJSP (link: tinyurl.com/4227ejhn) show how synaptophysin alone might not be the best screening marker in the dx work-up.

A nice case of Ochronosis. Banana-shaped, ochre-yellow (golden-brown) pigment in thickened dermal collagen bundles. Can be endogenous from alkaptonuria but more frequently exogenous origin secondary to hydroquinone treatment. #DermX#Dermpath#Dermtwitter#pathology#MedEd

Interesting variants of mucoepidermoid carcinoma. Part 1
- They can be ciliated
- They can have acinar differentiation (it is not acinar cell carcinoma)
- They can be spindled (usually keratin positive)
- They can be oncocytic (these were the old oncocytic carcinomas described in salivary glands which no longer exist)

Spindle cell hemangioma was described years ago as a "hemangioendothelioma", but is benign, often associated with vascular malformations, and occasionally with Maffucci syndrome (enchondromas and hemangiomas), described years ago.

45 yo female with a "bump" in tongue
Subsite not given
Pale intraepithelial structures (CK7 ) with underlying S100 spindle cells and chronic inflammation
Diagnosis: Normal taste bud (circumvallate papilla)
- Normal taste buds appear as pale structures on HE (CK7 , Pic 2)
- S100 spindle cells and chronic inflammation are expected underneath, Pic 3
- Most prominent in circumvallate papillae (posterior tongue)
- Do not mistake for Paget disease or neural neoplasm
