🍬 Insulinoma – The Most Common Pancreatic Endocrine Tumor ⸻ 1️⃣ What is Insulinoma? ➊ A rare pancreatic β-cell tumor that secretes excess insulin ➋ Most common pancreatic endocrine tumor ➌ Causes recurrent episodes of hypoglycemia ⸻ 2️⃣ Clinical Features ⚠️ Symptoms are due to hypoglycemia ➊ 😓 Sweating ➋ 🤝 Tremors ➌ 👀 Diplopia (double vision) ➍ 💪 Weakness ➎ 🍽️ Hunger ➏ ⚖️ Weight gain from frequent eating to relieve symptoms 💡 Symptoms typically occur: • Early morning • During fasting • Just before meals ⸻ 3️⃣ Key Investigations ➊ Blood Tests 📈 High insulin 📈 High C-peptide 📈 High proinsulin ➋ 72-Hour Fasting Test ✅ Positive if: • Glucose <40 mg/dL • Insulin remains inappropriately elevated ➌ Imaging 🖥️ CT/MRI pancreas to localize the tumor ⸻ 4️⃣ Important Exam Pearls 🧠 High insulin Low C-peptide ➡️ Suggests exogenous insulin use 🧠 High insulin High C-peptide ➡️ Suggests insulinoma or sulfonylurea use 🧠 MEN-1 association should always be considered ⸻ 5️⃣ Management ➊ 🔪 Surgical resection = Treatment of choice ➋ 💊 If not operable: • Diazoxide → inhibits insulin release • Octreotide → suppresses insulin secretion ⸻ 🎯 Key Takeaway Think of Insulinoma in any patient with recurrent fasting hypoglycemia, especially when accompanied by sweating, tremors, and unexplained weight gain. ⸻ 📚 Master Endocrinology with high-yield notes, illustrations, and exam-focused summaries! 🌐 Visit our website: mediconotes.com 📖 Download our Endocrinology Book today! #MedEd #MedicalStudent #Medicine #Me

(from Grok) 2026 Tumbler Ridge, Canada school shooting: Jesse Van Rootselaar, 18 (trans-identified female, biological male; identified as a girl since age 12), killed his mother and stepbrother at home, then went to the school and killed five students and one teacher. Died by 2025 Minneapolis Annunciation Catholic School shooting: Robin Westman (born Robert Westman), 23-year-old trans woman, opened fire during Mass, killing two young children and wounding others, then 2023 Nashville Covenant School: Audrey Hale (trans-identified, biological female), killed 6 people (3 children).factcheck.org Earlier: 2019 STEM School Highlands Ranch (Maya/Alec McKinney, trans-identified male); 2018 Aberdeen, MD workplace shooting (Snochia Moseley, trans-identified). Sexual offenses in prisons: UK Ministry of Justice data (2017-2019 snapshots): ~48-59% of trans prisoners (mostly MtF/male-born) had sexual offense convictions (including dozens of rapes), vs. ~3-4% of female prisoners and ~17% of males. Similar U.S. patterns show sexual offenses as the most common category for incarcerated trans women. On puberty blockers and castrations: Aggregate Data on Surgeries for Minors A Reuters/Komodo Health analysis of U.S. insurance claims (2019–2021) found 56 genital surgeries (including procedures like orchiectomy, vaginoplasty, etc.) among patients ages 13–17 with a prior gender dysphoria diagnosis. This is out of thousands of youth in gender care; most surgeries on minors are chest-related (e.g., ~776 mastectomies in the same period). No genital surgeries reported for ages 12 and under in key 2019 data. Guidelines (WPATH, Endocrine Society) generally recommend genital surgeries at 18 , but some clinicians performed them on older teens in "rare" cases with persistent dysphoria, parental consent, and assessments. Many hospitals/clinics restricted or banned them for minors. Specific named public cases of orchiectomy on minors are not widely publicized (due to privacy), but the aggregate numbers confirm they occurred. Orchiectomy is sometimes done standalone or as prep for vaginoplasty. Effects of Blockers Hormones (Functional "Castration" Without Surgery) Puberty blockers (GnRH analogs) followed by cross-sex hormones often cause sterility, infertility, and sexual dysfunction even without surgery — this is what some critics call "chemical castration." Blockers pause puberty; ~96–98% of youth on them proceed to hormones. Long-term: Reduced fertility (often permanent if followed by hormones), underdeveloped genitals (complicating future surgery), low bone density, and unknown brain development impacts. This pathway is far more common than surgery for minors. European countries (UK post-Cass Review, Sweden, Finland) have restricted it due to weak evidence of benefits.

Truth - but Focus on source. Atrazin and many other Endocrine Disruptors, damaged wide range of hormones - the Testosterone decline is just a proof, but the pair Bond via Oxytocin suffer too. Cortisol, chronic elevated by Stress, directly destroys Oxytocin Bond, major for woman.

This is an invaluable resource. Proving metabolic & pharmaceutical malabsorption to a primary care physician is notoriously difficult without objective data. This provides the exact empirical framework I’ve needed to demonstrate how these overlapping biological pathways intersect. It clearly illustrates the systemic domino effect occurring between glucose regulation, thyroid function, endocrine health, digestive acidity, cortisol production, etc… Thank you for turning lived experience into clinical evidence.
Another study shows cerebral hypometabolism in #LongCovid patients with PEM. Cerebral hypometabolism is a decrease in the brain's ability to turn glucose into energy, leading to a severe cellular energy deficit. journals.sagepub.com/doi/ful…

Your experience reminds me so much of a friend who also had a brutal time losing weight. For years she thought it was just her PCOS, but a doctor eventually discovered a hidden thyroid issue. Treating that was the missing key that finally unlocked her metabolism. Our endocrine systems are so interconnected, I always mention her story just in case it can help someone else find their missing puzzle piece too.

MYOSITIS MIMICS Not every elevated CK represents inflammatory myositis. A wide range of endocrine, metabolic, genetic, neurologic, toxic, and critical illness–related conditions can present with proximal weakness and elevated CK levels. Before diagnosing inflammatory myopathy, consider important mimics such as hypothyroid myopathy, muscular dystrophy, metabolic and mitochondrial myopathies, motor neuron disease, myasthenia gravis, drug-induced myopathy, vitamin D deficiency, and critical illness myopathy. The diagnosis of myositis requires more than an elevated CK. Clinical pattern, medication history, autoantibodies, EMG, muscle MRI, and sometimes muscle biopsy are essential. Which myositis mimic do you find most challenging in clinical practice? Old model: mass male labor, factories, unions, churches, ethnic neighborhoods, fraternal orders, street politics, local machines, veterans’ networks, shared hardship, high fertility, high physical congregation. If that heats up, the state may need war, draft, prison, strikebreaking, or frontier expansion. New model: atomized men, lower family formation, lower union power, lower civic participation, screens, porn, weed, SSRIs/stimulants, endocrine disruption, debt, credential gates, background checks, parole/probation, algorithmic feeds, gig work, obesity, disability, loneliness, low trust, and managed outrage. That population can be angry as hell while still being politically non-coherent.
