I deeply believe that we need biology to guide the development of the radiotherapy technologies. With emerging immunotherapy and molecular targeted treatments, we need to develop more precise and effective local therapy. More importantly, we should develop clinical, biology and image AI-guided individualized radiotherapy to work together with novel systemic therapy.

What is the future of particle therapy? Will we see better imaging, adaptive auto-plan optimization, and multi-ion therapy? I believe that we will never achieve a single “best beam” and will always need a “better” beam, utilizing biological and physical optimization along with image guidance. Radiotherapy is akin to satellite-guided missiles; there is always room for improvement in accuracy and effectiveness. Different ion has unique biological and physical features that can be used to tailor the “beam” to particular lesion. We are looking forward to the development of cost-effective, biology- and physics-based multi-ion systems in the near future.@PTCOG_Official

In head and neck cancer, two randomized studies comparing IMRT and IMPT (proton TX) were published in Lancet this year. #PTCOG64. Both studies showed improvements in some acute side effects, and one study found overall survival benefits favoring IMPT. Today's plenary session of #PTCOG64 Day 2 focused on the immunological mechanisms and potential advantages of particle therapy and flash therapy. These approaches may spare lymphocytes, function as in situ vaccines, and modify the microenvironment when combined with immunotherapy, offering greater potential for the future. @PTCOG_Official

For sure, we need to know more about the brain, the anatomy, functions and dose response for each specific structure. Patients survive longer, have more recurrence over years or decades and brain metastasis becomes more complex now.

When I was a resident 24 years ago, we were taught that renal cancer is radiation resistant disease. Now, we learn that RT can achieve up to 100% local control when RT dose is appropriate.
