Smith: “NSTEMI” is worthless diagnosis. 30-40% of NSTEMI are Occlusion MI, and they have far worse prognosis than NSTEMI with open arteries, in spite of being younger and healthier at baseline. NEVER diagnose someone with NSTEMI. It is a worthless diagnosis.  Very true✅

#AHA25 in the world of AF a lot of science was presented: 1) #OCEAN Rivaroxaban did not signif ↓ incidence of stroke, systemic embolism or new embolic stroke vs ASA among high risk patients for stroke post ablation 1-year prior @NEJM nejm.org/doi/full/10.1056/NE… 2) #ADAPT_AF NOAC monotherapy was noninferior to combination for net adverse clinical events in AF patients who had undergone DES 1-year prior @NEJM nejm.org/doi/full/10.1056/NE… 3) #OPTIMA_AF 1-mo was as effective as 12-mo dual antithrombotic therapy w DOAC & P2Y12 inhibitor, followed by the oral anticoag alone for preventing stroke, MI & death & fewer bleeding than long dual Rx 4) #CLOSURE_AF Physician-directed standard medical Rx (anticoag) was better than LAAC in patients with AFib & at high risk of stroke & major bleeding

💡 11/ Take-home: Post–cardiac arrest care may move beyond “one-size-fits-all.” A neurologically centered, tailored TTM approach (TTM-HT vs TTM-NT) may optimize outcomes.
