 Your staff will look for other jobs. You and your senior partners (financial beneficiaries of PE transaction) will lose motivation forcing younger partners/associates to pick up the slack Your younger partners/associates will resent you, but won’t be able to say anything due to power dynamics and fear of retribution from PE overlords You’ll lose your recruiting edge You will not practice medicine the way you want, you will practice medicine the way private equity wants Patients will say things like “why is everybody so grumpy here?” and “what happened to this place?” Instead, keep your practice, you’re doing great!

And a part of pre-onc treatment as well (knowing the likelihood of needing abx at some point during therapy). Many documented allergies are historic early childhood experiences that then relegate adults to 2nd or 3rd line empiric antibiotic management when facing ID crises.

It’s also hard to watch regulatory burdens necessitating ID expertise increase while salaries continue to be based on a (poor) clinical RVU model. We are the single best quality investment for a medical center but are not paid commiserate to that level of value (/necessity).
