Ketogenic diets historically have not been recommended for people with anorexia nervosa (AN), largely because of concerns about weight loss. However, a new feasibility study explored whether a weight-maintaining ketogenic dietary therapy could be safely implemented in adults with AN. Twenty-two women who were weight-restored or mildly underweight followed a ketogenic diet providing approximately 70% of calories from fat, 20% from protein, and 10% from carbohydrate for 14 weeks. Blood ketones were monitored daily during the first two weeks and weekly thereafter, with a target BHB range of 0.5–3.0 mmol/L. Key findings: 🔹 82% (18/22) completed the intervention 🔹 72% of completers no longer met the criteria for diagnosis of AN 🔹 72% of completers achieved depression scores within the normal range 🔹 Improvements were also seen in anxiety, self-esteem, body dissatisfaction, and mealtime distress 🔹 BMI remained stable, with no clinically significant weight loss 🔹 No serious adverse events occurred This was a small, uncontrolled feasibility study, so the findings should be considered preliminary. However, the results support further investigation of ketogenic therapy as a potential adjunctive approach for anorexia nervosa 💥 @GuidoFrank, Scolnick B, Beckwith C, Shott ME, Kilty S, Swindle S, Lutter M, Rho JM. 🚨 A recent randomized clinical trial compared different weight loss approaches in adults with obesity In just 3 months, several diets outperformed the classic Mediterranean approach for losing weight: 🥩 Ketogenic diet: −3.8 kg (8.3 lbs) greater weight loss ⏳ Alternate-day fasting : −3.1 kg (6.9 lbs) greater weight loss 🕒 Late time-restricted eating: −2.3 kg (5 lbs) greater weight loss 🌅 Early time-restricted eating: −1.22 kg (2.7 lbs) greater weight loss All diets reduced intake by 600 calories per day, fat loss was highest with mADF and early time-restricted eating, and there were no major changes in lean mass or heart health markers. 📉 Keto and intermittent fasting may lead to greater weight loss than a standard calorie-restricted Mediterranean diet, although long-term results require more study. — Martínez-Montoro JI, Bandera B, Gutiérrez-Bedmar M, Gómez-Pérez AM, Macías-González M, Moreno-Indias I, Tinahones FJ. Can carbohydrate restriction directly improve beta-cell function—independent of weight loss? In a 12-week randomized controlled trial, adults with type 2 diabetes followed either: 🥑 A ketogenic diet (~9% carbohydrate) 🍞 A low-fat diet (~55% carbohydrate) Participants received all foods and met weekly with a dietitian, and the diets were designed to maintain weight in both groups. The ketogenic diet led to greater reductions in a key marker of beta-cell stress (proinsulin-to-C-peptide ratio, or PICP): 📉 ~56% greater reduction in fasting PICP 📉 ~49% greater reduction in post-challenge PICP These reductions were associated with improvements in insulin secretion during hyperglycemic clamp testing. Additionally, body weight remained stable, suggesting that a ketogoenic diet without weight loss may help reduce beta-cell workload. This study adds to the evidence that lowering carbohydrate intake may support beta-cell function in type 2 diabetes 💥 — Yurchishin ML, Finn AR, Fowler LA, Vere-Whiting SL, Gower BA. 🔍 What happens to liver health markers when keto-adapted individuals switch to high carbs? A small study of long-term keto-adapted women (avg ~4 years) found that after just 21 days of high-carb intake: • ALT ↑ 85% • GGT ↑ 29% • Insulin resistance (HOMA-IR) more than doubled • ALT/AST ratio also increased All markers returned to baseline after resuming keto, suggesting reversibility. 📊 GKI closely tracked these shifts, highlighting its potential as a monitoring tool. Takeaway: Even short-term high-carb intake may disrupt liver-related metabolic markers in keto-adapted individuals. — Cooper ID, Petagine L, Soto-Mota A, Duraj T, Scarborough A, Norwitz NG, Seyfried TN, Furoni MA, Kyriakidou Y. 🚨 New randomized trial shows a personalized ketogenic diet can improve real-world function in McArdle’s disease, a rare genetic disorder that blocks the breakdown of muscle glycogen and makes exercise painful and fatiguing. In a 6-month single-blind RCT, adults with McArdle’s disease – also known as glycogen storage disease 5 (GSD5) – followed a ketogenic diet or maintained their usual diet. Key changes in those on the ketogenic diet: - VO₂ peak rose by 2.7 ml/kg/min - Walking distance in 12 minutes increased by 180 feet (55 meters) - BMI decreased by 2.3 kg/m² - No serious adverse events were reported - 5 participants continued the diet post-trial and reported sustained benefits 3 years later This is one of the longest ketogenic diet trials in GSD5 to date, supporting this dietary therapy as a safe, sustainable strategy to improve aerobic capacity and physical performance in this rare metabolic condition 💡 — Martinuzzi A, Musumeci O, Stefan C, Vinante E, Ferrati A, Perillo C, Pesenti N, Toscano A . Weight regain after stopping GLP-1 is common. But it may not be inevitable. One study found that people with type 2 diabetes who stopped GLP-1 therapy while following a ketogenic or low-carb diet: • Maintained weight loss after 1 year • Kept HbA1c below the diabetes threshold • Saw outcomes comparable to those who stayed on GLP-1 With the right strategy, long-term metabolic health is possible 🙌 Learn what to do next 👇 bit.ly/4cQn67X #MetabolicHealth #KetogenicDiet #LowCarb #Keto

💪 Can exercise intensity impact ketone levels during therapeutic keto? A recent case report suggests it can. In a glioblastoma patient, blood ketones reached ≥4.0 mM with high-intensity exercise, but not with walking or steady cycling on the same diet. The insight: intensity may play a bigger role than calories burned when aiming for deeper levels of ketosis. 📈 While more research is needed, this highlights an important consideration for those using keto as part of a therapeutic approach. 📌 Link to study: bit.ly/4tfkt4E 📌 Find more research summaries on our website: bit.ly/49p9WfU #KetogenicDiet #Glioblastoma #NutritionalKetosis #Exercise #MetabolicHealth
