As we close out 2025, we want to say a huge thank you to our entire community! In less than a year, more than 65,000 clinicians have come together to build one of the biggest and most comprehensive salary datasets in medicine. We now have 20,000 anonymous salaries across all 50 states, plus MGMA specialty snapshots, all free for verified clinicians. How do wRVUs vary by specialty? Physician compensation is increasingly shifting to wRVU-based models - but the #wRVUs and reimbursement rate per wRVU varies widely by specialty. The number of wRVUs you can generate largely depends on how CMS assigns Relative Value Units (RVUs) to different procedure codes. Unfortunately, Primary Care and related specialties like Pediatrics, Family Medicine, and Internal Medicine typically generate fewer wRVUs due to the nature of their high-volume, shorter duration visits. Adding to the challenge, reimbursement rates ($/wRVU) for these specialties are often lower since they rarely benefit from significant facility fees. This creates a double-whammy: fewer wRVUs at lower reimbursement rates. On the other hand, surgical and procedural specialties like Neurosurgery, Orthopedics, and Cardiology typically produce higher wRVUs at higher reimbursement rates, driven by higher complexity, longer duration procedures, and associated facility fees. Combine these factors, and the compensation gap between these specialties can be as large as 4x

Two key insights here - when there is a pay gap between Academics and Non-Academics, it likely means that clinical load will be lower, which could be great if that's what you are looking for. It also means that looking at overall averages is misleading. e.g., Allergy is great in private practice even with a full clinical load since the hours are great, almost no call, and highly competitive pay

📊 New Post on Marit: Nurse Practitioner Salary Deep Dive The median NP salary is $136K. But 1 in 12 Nurse Practitioners report compensation over $200K. We analyzed thousands of verified NP salaries to understand what characterizes these higher-earning roles: - Specialty: Most common in Dermatology & Aesthetics, Surgery, and Psychiatry - Employment type: 1 in 3 self-employed NPs earn over $200K - Compensation model: Over half of NPs on collections-based or net income models exceed $200K - Schedule: NPs working nights, weekends, or variable-length shifts are nearly 2x as likely to earn >$200K - Geography: Higher pay concentrated in Western states (particularly CA) and large metro areas. Worth noting that this differs from physician salary trends, where rural compensation is far higher that large metros. There’s no single pathway, but there are some common patterns in the data.

How do schedules impact NP Salaries, and Satisfaction? One of the more striking findings in our analysis of NP salaries on @MaritHealth was the relationship between shifts, schedules, pay and pay satisfaction! Tougher schedules do correlate with higher salary, but the magnitude of differences is quite meaningful. Let's dive in - - NPs working regular 8hr weekday shifts: $140K Average Total Comp - Weekdays weekends: $148K (~6% higher) - 12hr or variable shifts: $154K (~10% higher) - Day & night shifts: $154K - Night shifts only: $171K (~22% higher) - Nights weekend shifts variable lengths: $176K (~25% higher) What's even more interesting is - - NPs working 8hr weekday shifts report a pay satisfaction of 3.4★/5.0 - NPs working nights/weekends? 3.7★/5.0 These roles aren’t for everyone, but it’s good to see higher pay for these tougher schedules. We published the Top States for Physician Pay Satisfaction in 2025! Where you practice matters. Most physicians become deeply rooted in the communities they serve, and their satisfaction with pay is influenced by the cost of living, the lifestyle they can sustain, what it's like to raise a family in that location, in addition to whether their work feel valued in that setting. The correlation between compensation and satisfaction is only moderate, at just over 0.5. - Top States - Nebraska, Nevada, and Arkansas rank among the top states, with average satisfaction scores exceeding 3.8 out of 5. These markets, known more for community-based care and manageable cost-of-living, consistently outperform traditional "destination" states when it comes to how fulfilled physicians feel in their roles. - On the other end - Markets in the Northeast tend to have lower pay satisfaction due to a higher concentration of physicians, even as the cost of living remains high. California ranks near the national average for pay satisfaction, even though wages are significantly higher than in most other states. - The highest-rated metros for physician satisfaction include Las Vegas (4.0), Salt Lake City (3.9), and Cincinnati (3.84) Here are the top 20 highest and lowest ranked states and metros.
