And remember this when they come in middle of night looking fine and parents say they were “wheezing” without asthma/RAD history 1- they did look bad (stridulous) at home 2- the cold humid air outside made better 3- it was stridor not wheezing (parents only use words they know)

In patients presenting with cUTI (including acute pyelonephritis) who are improving clinically on effective therapy, we suggest treating with a shorter course of antimicrobials, using either 5-7 days of a FQ or 7d of Non FQ rather than 10-14d 14 days for cUTI is obselete!

I’m sharing an actual recent peer-to-peer call that shows what physicians and patients face when trying to get a surgery approved. This call felt as absurd as it sounds. The peer-to-peer call was to advocate for surgery to prevent and treat lymphedema for a patient with breast cancer. Her risk is high and we can perform a surgery to lower it. The doctors from the insurance company on the call were an ophthalmologist with a subspecialty in oculoplastic surgery and a plastic surgeon who currently has a cosmetic practice. Neither doctor has ever performed lymph venous bypass…the surgery I was trying to get approved for my patient. Neither doctor would provide their name or license number. At the conclusion of the call, the doctors told me that they didn’t have the power to make a determination or decision to perform the surgery. They were just there to inform me of United’s decision to deny it. The Medical Decision was not up to the doctors on the call. It had already been made by United. So here I am, appealing again. Medical decisions should be made by doctors who are well informed and patient-centered. Patients deserve access to the care that they paid for with their premiums.

RFK Jr. went on national TV and spouted egregious, dangerous falsehoods about vaccines. As a parent and infectious diseases doctor, I couldn't stay silent. @FoxNews might not fact-check him, but I will. I've reviewed the trials. I've catalogued them. I have receipts. 🧵

Exercise “Better Then Drugs” to Stop Cancer and Prolong Life after treatment! 🏃‍♀️ A groundbreaking study published in @NEJM found structured exercise after chemotherapy significantly reduces risk of colon cancer recurrence & death. The decade-long randomized controlled trial involved 889 patients from six countries who had completed chemotherapy for colon cancer. Participants who engaged in a supervised exercise program experienced a 28% lower risk of cancer recurrence and a 37% reduction in overall mortality compared to those who received only general health advice. The exercise regimen, tailored to individual preferences and capabilities, included activities such as brisk walking, jogging, and swimming, aiming for three to four sessions per week. Participants received coaching and support to help them achieve their exercise goals over a three-year period. Experts suggest that these findings could transform cancer care by integrating structured exercise programs into standard treatment protocols. Beyond colon cancer, the benefits of exercise may extend to other cancer types, offering a non-toxic and cost-effective strategy to reduce mortality risk and improve patient outcomes. 🔗 nejm.org/doi/full/10.1056/NE… #cancer #exercise #meded #exerciseforcancer #CancerRecovery #ColonCancer #CancerSurvivor
