Main issue is that we try to see an effect of an agent in a heterogenous population. ICU population is not homogeneous, that's why none (Except steroids) have shown any beneficial effect. Each tested therapy might be effective in a select population, but heterogeneity negates it.

Large observational studies show higher Vitamin D levels, and use of vitamin D supplements, associate with lower risk of breast cancer and lower risk of CV disease. You know what doesn’t prevent breast cancer or lower CV events? Vitamin D supplementation.
Women taking GLP-1 drugs (Ozempic, Wegovy, Mounjaro) were about 30% less likely to develop breast cancer, despite matching for age, BMI, race, diabetes status, & breast density. The more we learn about this drug class, the more it looks like we've stumbled onto something big.
