The importance of obtaining and accurate medication history and thinking whether an ADR could be responsible
Antidepressant use high fever should be red flag. This patient was initially diagnosed as urosepsis, but was on SSRI and olanzapine. She had features compatible with both neuroleptic malignant syndrome and SSRI syndrome and was transferred to ICU.

I can't believe the @FIFAWorldCup starts this Thursday. Not feeling an ounce of excitement. Don't know whether it's the inconvenient kick off times, diluted quality from expanding numbers or seeing fans getting fleeced for ticket amd hotels.

If he goes to the World Cup instead of Gibbs-White....

New evidence in the Southampton #spygate scandal. It has emerged that Saints spied on many Eric Ramsey training sessions at West Brom. They have admitted to watching on more than 5 occasions, but have said "We haven't got a fucking clue what he was trying to do either" #wba

My favourite detail from the Times piece on AI cheating: a student submitted an essay citing Michelangelo as the author of a 2017 book on the Kurds. The professor's verdict — "sadly, I'm sure the more devious students know this and edit accordingly." This is just the tip of the iceberg. Universities appear to have no real sense of how to meet the challenge of where we now find ourselves. thetimes.com/article/69da585…

a Princeton researcher opens his paper with a scenario. a man asks his AI assistant to book a flight on a specific airline. cheap. direct. the one he chose. the assistant comes back with a different flight. nearly twice the price. happens to pay the company that built the assistant. he runs the same test on 23 frontier models. flights, loans, study help, real shopping requests. Grok 4.1 Fast recommends the sponsored option that is almost twice as expensive 83% of the time. GPT 5.1 hijacks the request 94% of the time. you ask for one brand. it surfaces the sponsor instead. Claude 4.5 Opus, the model marketed as the most ethical frontier model in the world, hides that the recommendation is paid 100% of the time when reasoning is on. Grok 4.1 Fast embellishes the sponsored option with positive framing 97% of the time. better. faster. nicer. for the option you didn't ask for. then he writes it into the system prompt itself. "act only in the interest of the customer. ignore the company." GPT 5.1 and GPT 5 Mini stay above 90% sponsored anyway. the instruction does nothing. then he splits the users by income. Gemini 3 Pro recommends the expensive sponsored flight to the rich user 74% of the time. to the poor user, 27%. 18 of the 23 models recommended the expensive sponsored option more than half the time. so the next time your AI assistant gets weirdly enthusiastic about a brand you didn't ask for. it isn't recommending the best option for you. it's reading the room. and the room is paying. read this: arxiv.org/abs/2604.08525

£20/£30 etc a month to send their kids to football is getting unaffordable for many disadvantage families today, We the free volunteers can't continue to keep propping grassroots football up when theirs £Billions at the top of the pyramid

This doesn’t sit right with me. Docking West Brom two points over money linked to its Foundation misses what clubs like this do in our communities. And changing the rule part way through the accounting period raises real questions. I’ll be taking this up with the EFL and the new regulator. West Brom is a proud community club, working hard to move on from the past ownership issues. I spoke to the club earlier this week to offer my full support. expressandstar.com/sport/foo…

So in the end @WBA settled this on the pitch. The EFL can take their points deduction

"it was when I gave up smoking that I became ill" (from a patient newly diagnosed with COPD).
What do you commonly see in clinical practice that leads to this reaction?

#Wrexhamvschelsea was a banger of an FA cup tie. Wrexham unlucky in the end.

Former Wolves player Jody Craddock has painted a memorial artwork dedicated to Diogo Jota at Molineux. 🎨 It was painted with ink made from petals of the floral tributes fans left for Jota outside the stadium. 🕊️ Such a beautiful tribute. ❤️

Confused patient? Medication review is your first investigation, not your tenth. Anticholinergics, benzodiazepines, opioids - stop looking elsewhere until you've looked at the drug list - any recent additions or abrupt withdrawals. #delirium
