Optimising Staff-patient Communication in Advanced Renal disease (OSCAR) - 4-year NIHR-funded research study led by @Lucy_Selman. Follow for study updates!
Did you know that at some renal units about 5% of older people have dialysis, while at others 95% do? This is partly due to how clinicians communicate about treatment options. In this new blog I discuss how the @OSCAR_study aims to tackle the problem #CKD#decisionmaking
New research kicking off later this year will investigate how kidney doctors engage with & support older people making the decision whether or not to have dialysis. Found out more about the @OSCAR_study by reading our latest blog by @Lucy_Selmankidneycareuk.org/news-and-ca…
📢 New paper in @BMJ_Open led by @BarnyHole, from his brilliant PhD.

This paper from @OSCAR_study builds on the CKMAPPS study, particularly qualitative data analysis by @SKGTonkinCrine@paulrsoton and colleagues, by showing how treatment decision-making is biased by the framing of dialysis and conservative management.

In the second, more common approach, CKM is framed as subordinate. It is not presented as having potential benefit for the patient: the disadvantages of dialysis are not explicated, and neither are CKM’s potential advantages. See example Fig 4.

The NHS has a duty of candour.
That has to come from the top down.
Spinning. Misleading. Misrepresenting truth to patients. All are completely unacceptable.
Team members Dr Chloe Shaw and Dr Ryann Sowden are now back from the @ERAkidney conference in beautiful Stockholm, where they presented findings from @OSCAR_study on treatment decision-making in advanced kidney disease. 1/3
@RyannSowden presented on how descriptions of treatment options in information resources are often biased and how this may influence patient understanding and decision-making. 2/3
Dr Chloe Shaw presented Conversation Analytic findings regarding the presentation of treatment options to older patients. We found that clinicians tend to present dialysis as the default treatment & conservative management as subordinate. 3/3
Very original work by @Lucy_Selman on @OSCAR_study using conversational analysis. Revealed 2 ways of presenting renal treatment options - dialysis as normative (audio example in slide) vs. conservative management as valid option. #EAPC2023 👏🏻👏🏻
