The Trump administration is quietly moving to change the Affordable Care Act to let health insurers offer people loans to pay for their care. Deep in a document over 1,000 pages long about how the Affordable Care Act market will operate next year, the administration suggests that insurers consider offering loans to cash-strapped customers. Under this approach, people who develop a costly disease or need unexpected emergency care would turn to their health insurer for loans. A third of American households already have medical debt, and this approach would mean even more debt that patients owe to their health insurance companies. I told you they were coming for the Roadless Rule. Yesterday, Republicans made their move — and they hid it inside a wildfire bill. Here's what makes this so enraging: 59 million acres of America's wildest national forests are now on the table. The 2001 Roadless Rule has protected nearly 60 million acres across 39 states for 25 years. No logging. No road construction. No drilling. No mining. Built after 1.6 million Americans showed up — at 430 public hearings nationwide — to demand it. What lives here: bald eagles, elk, black bears, Cerulean warblers, marbled murrelets. Species that need large, intact, unfragmented habitat to survive. For many of them, roadless forests aren't just home — they're the last places left. What the amendment does: guts the rule. Opens the backcountry to logging and road construction under the cover of "fire prevention." The administration is pursuing repeal through the executive branch at the same time. And unlike the original rule — they aren't holding a single public hearing. 1.6 million people showed up to protect these forests. The administration isn't asking anyone this time. Secretary Kennedy insists his actions (gutting ACIP and slashing the childhood vaccine schedule) somehow “preserved access and coverage.” Regardless of how one feels about vaccines, the facts are important -and his explanation comes across as a sleight of hand. Insurers voluntarily pledged to maintain no-cost coverage (tied to the prior ACIP schedule), not because HHS required it, but because they anticipated the harm to American health if uptake dropped. And providers are clearly less likely to stock/offer vaccines shifted off the standard schedule- for both coat and legal reasons. Access and choice aren’t being "preserved" by his actions - they’re being propped up by others amid the uncertainty he has created. 🤷🏽‍♂️
Today we filed a motion asking the First Circuit to expedite our appeal of the district court's order in the AAP lawsuit that left ACIP—the nation's vaccine advisory committee—without a quorum. AAP opposes our motion. I've been consistent from day one: I do not want to take vaccines away from anyone. Our policy changes preserved access and coverage. But the court's order has left ACIP unable to carry out its core responsibilities. As a result, the committee cannot issue new recommendations, review newly approved vaccines, or complete important work ahead of the fall flu season. A functioning ACIP is essential to ensuring that vaccine recommendations remain grounded in evidence and available to the families and providers who rely on them. Families, physicians, insurers, and public health programs deserve certainty—not paralysis. That's why we're asking for expedited review. Our appeal seeks to restore a functioning ACIP so the vaccine recommendation process can continue, and families, physicians, and public health programs have the guidance they need.
