According to @LLSusa, only ~90K of 48M denied claims are appealed. So we're advocating for 1) DOL FAQ that urgent cases aren't limited to inpatient care & 2) if there's no response w/in the required 72 hrs, an urgent claim should be automatically approved. plansponsor.com/health-claim…
