Crash intubation in A&E the other night. Urgent , so we observe a shortened checklist. 20 seconds later we’re ready to RSI, having identified suction not switched on. If you think you don’t have time to do the checklist, the more you need to do it. #patientsafety

The Global Anaesthetic Safety (GAS) card has received great feedback in many African nations, distributed by @SAFE_courses and @wfsawca. We’ll soon distribute sample cards to anaesthesia societies and keen providers in LMIC. Pls DM me or @mack_kalengajr if interested. 🌍 🇬🇧🇫🇷

The checklist cards usually hang on the lanyard so they can’t be forgotten, and are carried by a whole team. But, some hospitals hang a single card on the emergency grab bag or pager. One card costing under £1, but one big impact. #patientsafety @dasairway @ICS_updates

Sadly I think so many are focussing on non-clinical skills to get their application over the line. When at this period it is so crucial to get clinical experience under your belt. I say this as an academic clinician.

The food shelves in our huge #NHS tertiary hospital. Been like this for 3 days. This is the only food outlet, since the main hospital canteen was permanently shut in favour of this @WHSmith /M&S portal. Pls sort NHS food&drink provision @wesstreeting

Wise words from @Capt_Dude - we’re obsessed with leadership in medicine and never really focus on followership. #GAMC2025

What tools do people use for operational / performance debriefing (not psychological debrief) after emergency scenarios? Keen to see any aide memoirs / crib sheets used to guide structured team reflection. 🙏 #medtwitter #humanfactors

Does NAP 7 show we’ve lost our way with human factors? A short piece I wrote for @Assoc_Anaes on #humanfactors - missed opportunities around debriefing, CRM and simulation, and the thorny issue of transposing aviation standards to our practice. #patientsafety

I’m now peddling my opinions & plugging checklist cards on BlueSky. It’s actually quite good. Although you will probably never see this post. 🤔🤫 #MedTwitter

This won’t surprise anyone who sees emergencies in cardiac lab. - Unfamiliar, unmarked kit & storage - Unclear objectives between cardio/anaesthetic team - Variable (!) comms - Horrible ergonomics Could it be improved with better standardisation, joint sim & debriefing?

Out now! More NAP7 Cardiology patients receiving anaesthesia -often an unplanned event -logistically high risk (remote & unfamiliar environment?) -medically high (>1% of anaesthesia activity vs almost 10% of cardiac arrests) -poor outcomes (Contrasts very much with cardiac surgery - also <1% cases vs almost 10% of arrests … but impressively good outcomes) Worth a focus… @Anaes_Journal @jas_soar associationofanaesthetists-p…

Interesting….
This is the reason London telephone boxes (the red ones with the windows) were banned . They were death traps because the elderly fainted inside from the heat, and then were prevented from falling to the ground because of the 4 walls. Every morning there were a few elderly dead people retrieved from around London. The cure for a faint, is to fully faint. @EM_RESUS

Spent a fascinating day at an aircrew CRM course, welcomed by @mike_sooty and @draken_europe 🙏. Lazy parallels between healthcare & aviation are common - but emulating nuances of personal reflection, openness in error & debriefing (good & bad!!) in NHS are v achievable. ✈️
