The inquest into the death of Chris Nota will begin next week.
Chris was 19 years old. He had an autism diagnosis, learning disability, epilepsy and experienced mental ill health.
He died in July 2020 under the care of Essex mental health services.

A SBC worker had stated the LA do not recognise PDA, 70% children with a PDA are not in school. If the LA do not accept PDA profile despite many NHS providers recognising the profile then families will have to fight for provison at tribunal @SendSouthend@BLSENDlaw@IPSEAcharity

I have 3 x stage complaints unanswered with Southend 2 now referred to ombudsman, 2 sar sbc stated they needed 3 months to do and have missed deadline on these are all linked in with the ehcps
La named type as ms, ms are refusing, 1 x eotas agreed at tribunal, 1 x medical tuition with psychiatrist stating mainstream had led to mental health deterioration, 1 x slt report stating all staff to be aet2 autism and slt embedded in learning ripped out after coproduction 1/2
2/2 1 x all slt provision put in f not able to be done by previous ms and they removed must be done by slt. 1 x 5yrs behind. New la notified mid June of transfer. Old la gave ehcps in original state. I forwarded all tribunal docs to new team prior to panel.
