✳️ The 🔑 takeaway from this study is straightforward 👉 initiate #SGLT2i early, ensure consistent therapy, & recognize that the protective effects are not sustained after discontinuation.
Flozinate early and persist with flozination.

Monotypic fibrillary glomerulonephritis is not a MGRS in the vast majority of cases.
In cases with monotypic fibrillary - do IgG subclasses, do par IF, and check SPEP/UPEP for evaluation of true clonality.

#NephJC proteinuria is a surrogate of a surrogate (eg. eGFR decline) which is itself a surrogate of clinical kidney failure…& there is no surrogate for safety. Our community must prioritise large trials wherever we can…our patients deserve better!
