A man in his 30s with sickle cell disease presented in vaso-occlusive crisis after flu-like illness. After transfusion of 1 RBC unit, his Hgb fell: 9 → 4 g/dL. Despite another transfusion IVIG/steroids, he remained anemic, hypoxic, and ↑ LDH. Dx: Hyperhemolysis Syndrome (HS).

🩺 Management: -Avoid transfusion -Immunomodulation to curb hemolysis: IVIG, high-dose steroids, rituximab, or eculizumab -Supportive care: hydration, pain control, DVT prophylaxis, infection management -If transfusion is essential → premedicate use extended antigen-matched RBCs
