Eitan Kugler shares findings from a single-center retrospective analysis of patients with newly diagnosed Ph ALL in CR1 who discontinued TKI therapy after remaining in complete molecular response (CMR) for varying durations (N = 17).
Median time on TKI was 65.2 months and median time to CMR was 3 months. TKI discontinuation may be feasible in carefully selected patients: 76% maintained treatment-free remission (TFR) at a median follow-up of 47.9 months. Patients with CMR >4 years before discontinuation achieved sustained TFR without relapse. Of the patients that relapsed (n = 4), 3 patients regained molecular response on TKI re-initiation (the 4th patient has not yet started therapy). NGS MRD undetectability at discontinuation identified patients with durable TFR.

Excited to see the updated results from KOMET-007 presented by Dr. Zeidan @Dr_AmerZeidan at #EHA2026 and grateful to have contributed to this important study.
Ziftomenib intensive chemotherapy (7 3) demonstrated encouraging activity in newly diagnosed NPM1-mutated and KMT2A-rearranged AML:
✅ CRc 96% (NPM1m) and 90% (KMT2A-r)
✅ MRD negativity 85% and 82%, respectively
✅ Median OS not reached in either cohort
✅ Low 60-day mortality (2% NPM1m; 4% KMT2A-r)
✅ Differentiation syndrome in <5%, all successfully managed
These data support the ongoing phase 3 KOMET-017 trial evaluating ziftomenib in frontline AML. #AML #Leusm #MeninInhibitor #EHA2026

Notably, only 3 patients relapsed, all with emergence of IDH1-negative clones, highlighting deep suppression of the target clone but potential evolution of alternative leukemic populations.

Poster: A multimodal AI foundation model integrating bone marrow biopsy, aspirate smear, and single-cell morphology demonstrated strong performance for distinguishing MDS from non-MDS cytopenias, achieving 93% accuracy in the held-out test set and 99% in external validation. Tri-modal ensembling outperformed individual modalities, supporting the potential of AI-assisted, standardized MDS diagnosis.
Key next steps include prospective validation, assessment across challenging borderline cases (CCUS/ICUS), integration with molecular data, and demonstrating real-world impact on diagnostic accuracy, reproducibility, and clinical decision-making.
