Guess the root cause - A problem involving a #Salesforce flow!
An admin built and deployed a record-trigger flow with multiple GET RECORDS and multiple UPDATE elements on the same set of records (non-bulkified). This flow in Prod significantly affected the system's performance.

A gap in SF governance: While the senior developer committee mandated a thorough review of Apex and LWC development, the declarative development, including flows, was not included. The flows were peer-reviewed by other admins with similar skill levels.
