SBRT (50Gy/5fx, MR adaptive) in locally recurrent PDAC. N=46 (7 w prior RT). 1 year LC 73%, median OS 19 mo. Ablative RT = feasible, safe in locally recurrent setting & can confer long term dz control. Great to see this data published - congrats @adapthyun & team! @OncoAlert

NOM short course ! Locally advanced after TNT and 25-30 in 5 to tumor and 30-35 to sidewall nodes had a CCR had a 72 percent CCR rate 🔥 Boost to primary did not lead to better CCR - perhaps while we know there is a dose response (OPERA trial) for locally advanced disease modern systemic helps considerably. Need to see the results of the German AIO trial - congrats to @adapthyun and the whole team 👏🏼👏🏼

NOM-ERA results just dropped! 🔥 #ASTRO25 Awesome work @adapthyun et al! Wondering if those who got the optional 30 Gy × 5 primary tumor boost had larger tumors or more adverse features since cCR and TME-free survival looked similar overall. sciencedirect.com/science/ar…

Patients deserve options, including shorter radiation courses. Agree 5-6 weeks of RT has best published rates of avoiding rectal cancer surgery so far, but all that time is hard on pts. NOM-ERA’s 1 wk of RT with boost was VERY popular with @UVMcancercenter pts. Eager for results!

Yup, I wouldn’t give up on SCRT so easily to go back to CRT without conclusive data. Keep going @adapthyun. I will also be excited to see what PULSAR with gross dz boost can do from @NiuSanford! Looking forward to seeing head to head of just 5x5 vs CRT.

Thanks for this paper! Not too long ago, treating pancreas tumors w duodenal invasion to 100 Gy (prescription dose)/5 fx would have been unfathomable. On NRG GI-011/LAP100, we will encourage 5 over 25 fx regimen - this paper suggests 5 fractions should be feasible for most.

We quantify successful coverage by PTV_opt coverage since we are adapting each day. Mandate that PTV is covered by 25 Gy (otherwise TPS will push most of low dose away from OAR as well). Recognize that not everyone has access to adaptive radiation therapy... 3/
