What does this have to do with underlining? Typewriters didn't have the ability to use bold or italic type. That was added by the printer, who had the ability to use different fonts & typefaces.
Authors still needed a way to indicate when text should be emphasized, though. 3/

So you would backspace and add an underscore under each letter you wanted emphasized. The typesetter would adjust the fiddly bits on the printing press to use an italic typeface.
These days we can just, y'know, use an italic font. 4/
