Great thread.
It’s out! It’s very exciting to see the final results of the US phase III RCT of IMPT versus IMRT for oropharyngeal squamous cell carcinoma. Data have been percolating for some time at conferences (and social media), and it’s great to be able to crunch through the actual paper. I think our community owes a debt of gratitude to the investigators for pulling this trial off. It takes passion, perseverance and just raw time and effort to make such a study happen, and the resultant information is really valuable. There is a lot to go through here, I’ll start with my final conclusion, which is that IMPT is A, but not THE, standard-of-care for oropharyngeal cancer. thelancet.com/journals/lance…

This paper reflects the ongoing misguided consequences of adhering to the outdated LNT model. Notably, the lead author discloses a COI with ALARA imaging. The time has come to re-evaluate the role of LNT in modern radiological and nuclear science. jamanetwork.com/journals/jam…

Lots of this regulation is caused by the LNT model (linear non-threshold model), which assumes that any bit of radiation is bad—stupid when the background has plenty of radiation. We can eliminate that.

People who speak confidently and put down others are not always the smartest of the bunch - example one is from @VPrasadMDMPH re: alcohol. Observational data on health effects of alcohol consumption are unreliable, confounded, and “a large, multi-arm RCT is the only path forward”

The only path forward: a 7-arm RCT consisting of either decreasing, increasing, no change, or uncontrolled consumption of alcohol. RCTs are the gold standard for medical research, sure, but is this the only way to thwart pesky self reported data?

Big daps to @cameronatharp for generating the original APRT show idea then facilitating continued discussion around what can be a controversial topic. While there is an explosion of pubs showing what an APRT *can* do, few are discussing what they *should* do in the US.
