What a day on the Hospital at home team! We have successfully managed general infections, pneumonia, atrial fibrillation, hypertension, angina & heart failure. Meaning, we've helped our patients to get home sooner and receive the hospital care they need safely in their home. @

An update after the long weekend. 56 (!!) referrals received, mostly from ambulance but also 111, acute trust and other community teams. 8 inappropriate but others all sorted. And still only 26 on list this am. That’s more than a ward full offered an alternative to admission!

Another day in a frailty #virtualward. A busy caseload of frail patients, often in care homes, for visits and reviews today. And now more great referrals from local ambulance offering alternatives to hospital. This really is the best thing I’ve done! @SECAmbulance @NHSKentCHFT
