Assessment of diastolic dysfunction remains one of the most challenging areas in echocardiography. Our new manuscript highlights key challenges, practical solutions, and common misconceptions in diastolic function evaluation. academic.oup.com/ehjcimaging… One of the central concepts is the “diastolic dysfunction framework,” which places the traditional echocardiographic definition of diastolic dysfunction into a broader clinical and pathophysiologic context. @JGrapsa @EHJCVIEiC

A 32-year-old female patient had her aortic valve replaced with a mechanical valve two years ago. She continued to experience exertional dyspnoea. A stress echocardiogram revealed that the mitral subvalvular lesion must already have been significant at the time of surgery. #echofirst

This finding was occasionally observed during dobutamine stress echocardiography. We suspected multi-vessel disease, but catheterization did not confirm it.
The interesting aspect of stress cardiomyopathy is often 'non-physiologic' symmetry. In this case of biventricular involvement, it appears as if someone drew a line between functional basal segment and non-functional mid/apical segments for both ventricles
