Many ‘long’ CTOs are actually several short CTOs in series. 🤔Sometimes there is a concealed vestigial TL within the CTO body. 👇🏼This case highlights use of multiple micro injections. It pairs nicely with @Laserrman’s case from EuroACTOS 2025.

The latest data shown in EUROACTOS 2025 amsterdam 3 days ago was while doing #HDR proximal Cap EP puncture happened in 30% of cases .. that number needs to come down .. So only 2-3 ways which come to head , (Either directly see where the is CAP) 1. To visualize the proximal cap and then puncture it , i.e IVUS guided ANTEGRADE WIRING ( very difficult to do ) 2. Make better educated guess by 3-D wiring ( again not commonly practised ) 3. Change the usual wires choices for proximal cap puncture ( since the usual prox cap puncture wires are going 30% extraplaque )
