Absolutely. Proactive planning will make a huge difference when recognised that a person is living with moderate and severe frailty. Opens up discussion with person and their family to ensure in the emergency situation we do what to they want. #frailtysensitiveapproach

We all doubt ourselves & our decisions at times. Believe me! If we don’t we risk being arrogant & inflexible. No HCP can afford To be like that. One size doesn’t fit all when using a #FrailtySensitiveApproach. Agree about support structures - that’s why we need MDTeams.
