Dr. Abhishek Shukla discussed conditions under which testicles are removed, with the most common reason in adults being prostate cancer. Since the testicles are the primary source of testosterone , the male hormone that can stimulate the growth of prostate cancer, removing them helps in slowing disease progression. This medical procedure is called orchiectomy, and it significantly reduces testosterone levels in the body. There are two types of orchiectomy: surgical and chemical. In surgical orchiectomy, the testicles are physically removed, while in chemical orchiectomy, medications like LHRH agonists are used to suppress testosterone production, both fall under Androgen Deprivation Therapy (ADT). Dr. Shukla also highlighted that while effective, this therapy comes with side effects such as loss of sexual desire, erectile dysfunction, hot flashes, and fatigue. He further explained a specific surgical method known as High Inguinal Orchiectomy (HIO), which is primarily done in cases of testicular cancer. In this procedure, the testicle is removed through an incision in the groin rather than the scrotum to avoid spreading cancer cells. If left untreated, cancers like prostate or testicular cancer can eventually spread to organs such as the liver, bladder, bones, and kidneys.
