Just read a great analysis from the SEQUOIA-HCM trial looking at the efficacy of aficamten, a selective cardiac myosin inhibitor, across different geographic regions ๐ŸŒ. Despite clear differences in baseline characteristics between Europe, North America, and China, the dosing, safety, and treatment effects of aficamten were remarkably consistent everywhere. Improvements in exercise capacity, LVOT gradients, and symptoms were seen independent of region โ€” which is a big deal when we think about how diverse HCM populations really are. A new study from a high-volume HCM center compared alcohol septal ablation (ASA) and transaortic septal myectomy in patients โ‰ฅ75 years with obstructive hypertrophic cardiomyopathy. Using propensity-matched cohorts (95 vs 95 patients), the authors looked at survival, LVOT gradients, and need for reintervention. ๐Ÿ” Key findings: ๐Ÿ‘‰ Procedural risk was low for both strategies (operative mortality 0% with ASA vs 1% with myectomy) ๐Ÿ‘‰ Complete heart block requiring pacemaker was significantly more frequent after ASA (23% vs 10%) ๐Ÿ‘‰ Survival was similar between groups and comparable to an age- and sex-matched general population ๐Ÿ‘‰ LVOT gradient reduction was more complete with myectomy, both early and at late follow-up ๐Ÿ‘‰ Reintervention was more common after ASA (trend toward higher rates) ๐Ÿ’ก Take-home message: In elderly patients with obstructive HCM, both ASA and surgical myectomy are safe and effective when performed in experienced centers.
