Hey #EPEEPS How's this for a circuit?!? Atypical Afl with a long narrow channel in a very low voltage area. EGMs spanning the full TCL were imperceptible on the recording system, but not on Grid with Nearfield-OT. Frequency based mapping annotates the critical isthmus with almost-perfect accuracy. Less than a handful of bad points in an 18k point map, voltage cutoffs of 0.1 mV would've hid the majority of this isthmus on the map. The propagation map below has voltage cutoffs of 0.02 mV. Voltage, however, is still useful in conjunction with frequency mapping. The whole channel lights up with low voltage, high frequency emphasis giving us a discreet target. Last case of the year and redo AAFL. Patient had prior Maze, PVI CTI line, and most recently a plethora of LA flutters. Went straight transseptal this time, thinking it had to be a recurrent LA circuit, but after extensive mapping, we found we were in the wrong chamber. #EPeeps

We saw from the map that flutter could terminate with either a roof line or floor line, and due to abnormal posterior wall voltage, decision was made to perform PW isolation. We started with the roof line, which slowed the flutter by nearly 100 msec before terminating. βš‘β€οΈβ€πŸ”₯

Sparklemap with Omnipolar arrows! I don't always use this feature, but it's pretty cool that each arrow direction is calculated w/out using LAT or a timing reference. I could set up the map completely wrong and the arrows would still point in the right direction. #OnlywithOT
