街医者やってると「この人そろそろCKDぽいけど、どこまで自分で管理して いつ腎臓内科の先生にお願いすべきか」で毎日地味に迷うことがあって。 KDIGOガイドラインは読んでるけど、毎回電カル開いて年齢・Cr履歴・ K・Hb・尿蛋白を全部照合するのが面倒でサボりがちなのが正直なところ。 実験的に、電カルの別タブで動くeGFRトレンド表示を作ってみた。 操作は基本3クリックで、 ① CLINICSの患者カルテでブックマークレットをクリック ② MedBoard(ダッシュボード)で取り込みクリック ③ eGFRトレンドにスイッチ 年齢/性別/Cr値の手入力は一切不要。既存のWebツールだと 毎回フォームに打ち込む手間があって結局開かなくなるので、 そこを潰したかった。 中でやってることは ・PrimePartner(健診PACS)から過去5年分のCr/K/Hb/BUN/UA/尿蛋白履歴を自動抽出 ・日本人版eGFR式 (194×Cr^-1.094×Age^-0.287, 女性×0.739)でプロット ・KDIGOステージ(G1〜G5)で背景色分け ・同年代の基準曲線を点線でオーバーレイ ・線形回帰でslope(年間低下速度)を計算 ・腎臓内科コンサルをお願いするかの判定を「医師モード」オーバーレイで表示 ・患者モードに切替えると医師判定パネルと左の詳細を全隠し →画面共有しながら患者に説明できる コンサル判定の閾値はKDIGO/日本腎臓学会CKD診療ガイドライン2018通り。 eGFR<30/slope<-5/年/K≥5.5/蛋白尿2 &eGFR<60 → 強くお願いする、 eGFR<45/slope<-3/年/K≥5.0/Hb<11 → お願いを検討、という普通の基準。 AIというよりただの計算機寄りのツールで、 ガイドラインを毎回手で照合する作業を自動化してるだけ。 で、仮想患者データで使ってみたら早速2件バグが出て、 1件目: 半年で2回しかCr測ってないデータ(97.2→72.2, n=2)で試したら 「年-52mL低下、急速進行、腎臓内科コンサル強く推奨」と出た。ただ、2点で線形回帰を信じて年換算はちょっといきすぎかなと。 誤解のないように書いておくと、Cr 0.64→0.84 (eGFR 25低下)自体は普通にびびるレベルの変化で、流していいやつではない。 急性変化(脱水/NSAIDs等)なのか本当にCKDの進行なのかの判別ができていないだけで、見逃していい差ではない。 じゃあ非専門医として何をすべきかというと、1ヶ月後に再採血して3点目を取り、slopeを再計算するのが私的には正しい手順。 3点揃えば信頼できる傾きが出せて、「本物の進行」なのか「一時的な変動」なのかを判断できる。 2件目: 補助項目に「Hb 6.6 g/dL」と出た。犯人はHbA1c(糖尿病の指標、正常4〜6%)を正規表現で誤拾い。「Hb」で始まる検査項目はHbA1cも含むのを忘れていた。 修正したのは、 ・slopeの線形外挿は n≥3 かつ観察期間≥6ヶ月 のときだけ KDIGO基準(-3/-5)を適用する(=信頼性ガード) ・HbのregexはHbA1cを除外 4〜20 g/dL範囲外は無効 ・ただし「2点でも下がってる事実は重要」なので、 絶対変化量ΔeGFRは事実として注意フラグ残す (例: 短期間でeGFR≥10低下, Cr≥0.2上昇) ・actionは「1ヶ月以内にCr再採血して3点目を取得→slope再評価」 ・急性変化の鑑別に「脱水/NSAIDs/採血条件」も並記 ガイドラインの閾値自体は1つも触ってない。 「閾値は正しい、でもその閾値を当てるデータが信頼できない時に黙って適用するな。代わりに必要な追加データを明示しろ」という設計に直した。 意図としては、こういうツールで一次照合と"次に取るべきデータ"の ナビゲーションを自動化すれば、非専門医でもガイドライン通りの 管理が責任持ってできるようになるかなと思ってて。 腎臓内科の先生にお願いすべき人を見逃さないのと、 まだ自科で管理できる人を不安で必要以上にお願いしてしまわない、 両方に効いてほしい。 ツール自体は判断の代理じゃなく、品質担保と次アクションの 補助装置っていうのが個人的に一番しっくりくる位置づけ。 閾値のチューニングや、尿蛋白定量(g/gCr)への対応とか、 詰めきれてないところはまだある。
