The PrecisionLife Study on MECFS showed disturbed CLOCK genes which regulate function through the SCN. A recent preliminary study shows deficient norepinephrine. Whether by genetic or epigenetic fluctuations, the CLOCK is an important regulator in MECFS, especially after an illness or other disturbance of homeostatic stability. "The SCN through both direct neural projections and indirect multisynaptic pathways, communicates circadian timing information to other regions, including the locus coeruleus (LC), the dorsal raphe nuclei (DRN), and the tuberomammillary nucleus (TMN), which releases histamine (Van Drunen and Eckel-Mahan, 2021; Scammell et al., 2017). Within the arousal network, the LC release norepinephrine to promote wakefulness and cognitive alertness, while the DRN releases serotonin, contributing to transitions between wake and NREM sleep. The TMN provides histaminergic drive that sustains cortical activation during wakefulness. The ventrolateral preoptic area (VLPO), by contrast, exerts GABAergic inhibition on these arousal-promoting regions to initiate and maintain sleep. Circadian SCN signaling to both the vLPO and LC-TMN-DRN axis ensures that sleep pressure is expressed at the biologically appropriate phase."
