How low can you go with HTN treatment in elderly?
Pick robust elderly: they’re heterogeneous group
Lowering SBP to 110-130 mm Hg ⬇️ CV events by 26% c/w standard treatment w target BP 130 - 150 mm Hg
No orthostatic hypotension in lower BP Rx group
#STEPstudy : In older patients with HTN , intensive Tx with a SBP target of 110 to less than 130 mm Hg resulted in a ⬇️ incidence of CV events than standard treatment with a target of 130 to less than 150 mm Hg

➡️Mean age 66 (75% below age of 70)
➡️ Most with well controlled BP at entry
Context of patient heterogenity
🫀Signif impact on CV events
🫀 Tx well tolerated & achieved! (SBP 126 Intensive)
🫀Is this SPRINT? BP achieved similar to STEP

Can intensive tx ⬇️ CVD risk risk HTN, older 👴🧓
🫀Optimum BP remains uncertain in older pt: differing rec's worldwide
🫀🇨🇳 age 60-80y, 9624 pts RCT, >50% Home BP cuff provided
🫀Intensive 110-130 SBP vs 130-150: 26% benefit with int tx

* Prediabetics had fall in HbA1c in tx vs placebo
* almost 1/2 of prediabetics normalized with semaglutide
*SF-36 physical and mental component & IWQOL-Lite-CT physical function improved with semaglutide vs Placebo

Thanks @drsebheaven !!
1. Maddie, MS IV at Penn State applying ortho
2. Inspired by being able to scrub with some incredible surgeons & being reminded why I love orthopaedics after being stuck in StepStudy/Quarantine land for 6 months
