A great @NIHRresearch scholars session this week, when I finally got a chance to meet colleagues I've only seen in the context of Outlook emails! Some fantastic sessions on implementation and building resilience, and I particularly enjoyed challenging my perceptions with a sociological way of thinking when approaching healthcare research We also had a great talk from the director of the #NIHR Research for Patient Benefit (RfPB) Programme, Prof Kevin Munro #NWRSP5 also enjoyed a gentle debrief🍺 afterwards. 📝 An eye on audit: Our annual David Wong Audit Symposium Prize Our junior doctors, foundation year doctors and even two medical students all submitted their abstracts for this annual prize event, which took place yesterday 11th September at the conference center in the new @LivHospitals The objective is to showcase exceptional audits, and foster collaboration, across the Mersey region. We want to highlight areas of good practice, recommend improvements, and for this event to serve as a platform for sharing action plans and proposed changes. More than that, we aim to empower our trainees, #foundationyear doctors and medical students to lead the way in improving patient care. 🤲 Described by our clinical director Mark Batterbury as "amongst the highest quality we've ever had", it was a resounding success as all nine entries were delivered with aplomb, and well-received by a captivated audience. The contenders were judged by a panel of senior clinicians from @LivHospitals, including the esteemed Prof Wong himself. 🏆There were two prizes in the symposium - one for the greatest impact in clinical practice, awarded to Zhihang Cheng (Effectiveness of local anaesthetic in #squintsurgery) and another for the best presentation, awarded to Jae Yee Ku (introduction of a patient information leaflet with pictures and videos to improve patients’ certainty of their posturing instructions after #vitreoretinal surgery) - I enjoyed editing Jae's videos and they are available at the bottom of this tweet. Both award-winners were presented with the hallowed symposium scroll - see picture of scroll's bestowment and contents below. ‍🎓A very special well done to the two @LivUni medical students who presented, which was novel for the symposium. Kathryn Orobosa-Ogbeide, at very short notice, presented her audit on impact of ruthenium brachytherapy plaque removal pre-planning on dose accuracy in choroidal #melanoma treatment, and Timothy Boey presented on screening for #AxialSpA in #HLAB27 associated #Uveitis, auditing patients presenting to the #Ophthalmology clinic. For both students, we hope the experience with be invaluable. 🏝️ The eye isn’t an island floating about on its own in the ether…. It is intrinsically linked to the rest of us. Systemic Diseases (and their treatments) can affect the eye. 🚨 How much do you know about immune checkpoint inhibitors (ICIs, or ICPis)? We are riding the crest of a wave in these exciting developments in #cancertherapy, which have revolutionised the fight against cancer. However, they do have side effects which affect other parts of the body. Alongside our colleagues in other departments, we join in the fight against immune-related adverse effects (irAEs), striving to ensure that our patients still get their life-saving treatments. 🆕 Here is another fine paper from our prolific Matthew Fenech in Orbit – deftly presenting a dyad of #lacrimal gland inflammatory complications of immune checkpoint inhibitors, a finding that (to the best of our knowledge) hasn’t previously been described! tandfonline.com/doi/full/10.… Our co-authors are Ayodeji Ajanaku, James Hsuan , Austin McCormick, Simon Shamas. We hope this adds to the growing body of knowledge on the complexities and challenges of these immensely promising treatments. 👨‍🏫Matthew is presenting an e-poster of this topic at the 42nd @esoprs meeting in Rotterdam next week (esoprs.eu/), and plans to expand this with a national study. This is an apposite continuation to the work of our @IvanSychev, who presented a large series of neuro-ophthalmic complications ICIS/ ICPis, for the @EunoSsociety meeting in the same city.  Hull-based pop-rock outfit #thebeautifulsouth did proclaim “It could be Rotterdam, or anywhere…" but looks like it’s mainly Rotterdam at the moment (Apart from Liverpool or Rome). 🛟 These groundbreaking treatments are changing #cancer and saving lives, and new therapies are on the near horizon. However, we will all need to be aware of the spectrum of adverse events - whether affecting eyes, skin, GI tract, liver, endocrine glands, lungs. Early recognition & management of these irAEs are vital to prevent severe or life-threatening outcomes. ⚕️Next year at our annual Medical Ophthalmology @MOSUKNews meeting, " In with the new but not quite out with the old"😊,  our late-morning symposium will be themed on #ophthalmic toxicity related to immunotherapy. So far, most of these have been in the area of #OcularInflammation/ #Uveitis, but as Matt and Ivan have shown us, there are many other possible manifestations. We look forward to an oncological perspective from @AOlssonBrown, and I will cover, as best I can, the ocular manifestations of #immunotherapy and targeted therapy. 🎨A Palette of Possibilities: Uveitis North-West Training Day at the Whitworth Art Gallery, Manchester, 3rd July 2024 🖼️The fusion of art and medicine creates a real tapestry. At the @WhitworthArt ni Manchester, we hosted the fourth meeting in our series which aims to foster a comprehensive understanding of uveitis, its complexities, and the latest advancements in treatment & management. 🖌️The gallery provided a serene and culturally enriching environment. Its walls, adorned with both historical and contemporary artworks, served as a symbolic backdrop for the day's theme - in particular the intricate and delicate artistry involved in diagnosing and treating ocular inflammation. 🧑‍🎨 Our canvas had four speakers today (#Uveitis consultant Joanne Wong orchestrating excellently, I could finally take a step back as deputy chair). The works in the gallery, particularly those of Caribbean-British artist & poet John Lyons, and Shirley Craven's vivid textiles reminded us of the importance of innovation, #empathy, and a #multidisciplinary approach when managing ocular #inflammation, painting a hopeful future for patients and practitioners alike. ‣ After I gave a "The Year Ahead" preview for upcoming #Uveitisresearch/events, our 4 artists peppered their well-crafted sessions with interesting cases. ‣ The first session was led by Guillermo de la Mata, #MedicalRetina consultant @ManchesterREH, delving into the fundamentals of Indocyanine Green angiography and providing intriguing cases that demonstrated #ICGAngiogreaphy in real-world scenarios, including a deft teasing of #whitedotsyndromes with these angiographic allies. ‣ Following this, Ajay Patil, Uveitis/ #NeuroOphthalmology Specialist @uhbtrust discussed retinal #vasculitis in a neuro-ophth context. Through occlusive and non-occlusive cases , Ajay shed light on the critical link between #retinalvasculitis & systemic diseases including #Behçet's disease, #sarcoidosis, demyelinating disorders & more. ‣ Then, Eleftherios Agorogiannis, Uveitis consultant @ManchesterREH, brought a dynamic and interactive element to the day with his emergency #uveitis session. Presenting 10 challenging cases peppered with vivid images, he actively engaged the trainees. ‣ Finally, Helen Devonport, consultant ophthalmic surgeon @BTHFT concluded the presentations with an informative & inspiring canvas of White Dot Syndromes. 🙏Thank you for our engaged trainees, fellows, speciality doctors, nurses and other ophthalmic practitioners - these days couldn't be successful without all of you championing the beauty of medical science and the artistry involved in providing our care through a broader, more compassionate lens. 🚀Liverpool leading the nation in clinical trials targeting IL-6 pathways 🚀Picking the Jabuticaba🍇 St Pauls' Eye Unit @LivHospitals is leading the charge in clinical trials targeting Interleukin-6 (IL-6) pathways - a crucial player in inflammation and immune regulation. IL-6 is produced by various cells in response to inflammatory stimuli, playing a vital role in B cell differentiation, T cell influence, and the production of acute-phase proteins. With IL-6 targeted therapies showing promise for autoimmune diseases and certain cancers, we're investigating its potential in treating non-infectious uveitis and #thyroid eye disease. Elevated IL-6 levels contribute to the inflammation damaging ocular tissues in uveitis and play a significant role in inflammatory milieu within the orbit. By targeting #IL6, we hope to reduce #inflammation, improve patient outcomes, and enhance the quality of life for those affected by these conditions. 📈 Our Achievements: ‣ We have recruited the first patient nationally and already exceeded targets in our uveitic #macularedema trial, where I serve as Principal Investigator. ‣ We have screened and recruited more patients for a #thyroideyedisease trial than the rest of the country combined. I am Chief Investigator for this @genentech @Roche clinical trial 📅 Just today, we have welcomed four clinical trials patients to our Clinical Eye Research Centre (CERC) today. This progress is a testament to the hard work of our entire CERC team, and our #research nurses Darina Koneva, Sandra Taylor, Florina Caban. Special thanks to our multidisciplinary clinic team, especially my joint orbital clinic with Austin McCormick (bit.ly/3VSoy1h), and our #Uveitis service with fellow consultants @navbeare, Nicholas Beare, Savita Madhusudhan and Shi Tan, our trainees, fellows, and our #ophthalmic pharmacists. Finding new perspectives when it comes to communicating research with my fellow NIHR research scholars last Friday at the Liverpool Science Park, adjacent to the magnificent @LiverpoolMet cathedral. Our objective was to unlock and practice ways of communication our research to patients and the public. Within our short arts and crafts exercise (this was always my worst subject at school) I attempted to turn my ethos of medical ophthalmology research and clinical practice into art: a single eye, brimming with vibrant, shining possibilities at its centre - each spark symbolizing our quest for clues to link the eye with systemic disease through research and innovation (!) OK, I wasn't very well so please excuse my slightly delirious creation. BOPSS, Leeds June 2024 🎉One of the busier and more intellectually stimulating part of my working week is the Joint Orbital Inflammation clinic I run weekly with my talented colleague, Austin McCormick. 🤞 I had promised him that I would visit a @bopssnews meeting, and had no excuses not to attend the Update & Recent Advances Day in Leeds, a city I previously lived and worked in for years, hosted by my little brother Sina Ghadiri. The event was based at the @carriageworks_ theatre in the @ElectricPressLS building complex (I last entered this 12 years ago for a very awkward #Kizomba class🕺🏽), with posters and refreshments at @LeedsCityMuseum and social at the magnificent Tiled Hall of the @LeedsArtGallery 📚 Despite being the only ophthalmic physician within a gaggle of #Oculoplastic & Orbital Surgeons, there was easily enough learning to be absorbed from an extensive overview of the latest developments and breakthroughs in oculoplastic and orbital surgery, in a day filled with engaging presentations and interactive sessions led by renowned experts in the field. 🔬 Topics ranged from innovative surgical techniques to advancements in patient care, and overviews on the latest oculoplastic-perspective management in Ocular Cicatricial #Pemphigoid (Saul Rajak) and #ThyroidEyeDisease (Jimmy Uddin). We are in a golden era for #clinicaltrials in TED, and there is a lot of optimism that our management of this challenging disease will move forward. I hope that my field, Medical Ophthalmology, will be key here, working alongside our Oculoplastic/Orbital colleagues. 🗣️It's always delightful to hear Geoffrey Rose's stimulating talks - a pair of sessions on management of functional #epiphora and of #canaliculitis. Intriguing talk on optic nerve sheath fenestration in #IIH by @drsachinsalvi 👥 One of the best aspects of the meeting is reconnecting with colleagues and friends from the #oculoplastic and #orbital community, and I couldn't have predicted how many friendly faces I would see there. It was a very collaborative atmosphere as we shared experiences, discussed challenging cases, and hopefully I promulgated the value of #medicalophthalmology alongside our fantastic surgical colleagues - striving to advance our collective mission to enhance patient outcomes. Our current TSC trainee Matt Fenech brought his excellent orbital #KimuraDisease paper to the meeting - watch out more fascinating papers from him coming soon, and also from Ayodeji Ajanaku. Within our unique service, we encourage our trainees and fellows to share the learning from our patients' cases, and thank our amazing patients for allowing us to share and collaborate with our colleagues nationally and internationally. 🍛The culinary highlight was the Thai green curry. I didn't get a chance to try the jambalaya but it certainly looked pleasant. #NIHR RSP Writing Retreat - June 2024 Foxgloves were well and truly out as we spent a couple of days writing in one of the finest residential libraries in the world – Gladstone's Library in North Wales 🏴󠁧󠁢󠁷󠁬󠁳󠁿 We @NIHRresearch Scholars Cohort 5 spent three days grant-writing as we bid to get some of our early-stage ideas and projects into fruition, to impact our patients and the future of multidisciplinary healthcare research in the UK. Founded by Britain's Liberal Prime Minister of 12 years, the #library is dedicated to dialogue, debate and learning for open-minded individuals. William Ewart Gladstone (1809-1898) was a political titan, switching from the #Tory party to the Liberal party and subsequently serving as Prime Minister four separate times during the #Victorian era. #Gladstone believed in education – he was a scholar, orator, and lifelong bibliophile who amassed an immense personal library of over 32,000 printed books.  After his death, the bulk of his remarkable book collection was bequeathed to establish Britain's first prime ministerial library and nation's first residential library, now known as Gladstone's Library – and a Grade I #listedbuilding. Located in Hawarden, Flintshire in Wales, just a few miles from Gladstone's boyhood home, the library opened its doors in 1906 and for my money is one of the world’s finest residential libraries, with a quarter of a million printed materials across a wide array of subjects. Our retreat was facilitated by Dr Steven Lane and Dr Mark Goodall, two fine fellows who helped us tweak and expatiate our applications.  Many of our cohort are allied health professionals, and for me it’s been great seeing the research appetite in our greater #multidisciplinary teams in the region. We shared ideas and thoughts on each others’ projects, fleshed out our narratives, engaged in informal peer review, then went to the reading room to work in the afternoons and evenings without distractions and in peaceful surroundings. As I was working, I looked up and saw some classic tomes of #Persian poetry, and also promised myself that at some point I’ll work my way through the many volumes of Fishes of the British Isles. The library complex houses a restaurant, lounge and a number of bedrooms. Once tired, my bedroom was only a few steps away from the library reading room. There were a number of interesting portraits in the residential corridors and other spaces (photos of Mkrtich Khrimian (Patriarch of the #Armenian Church and looks a serious figure) and Gregory of Nazianzus). There was also a crow resting on the lawn who watched us for the entire three days, ensuring that we didn’t dilly-dally and focused on aims and objectives whilst at this beacon of intellectual pursuit and literary heritage.
