The war mongers are in Tehran, not Washington. Between 2009 and 2025, the United States spent twelve years under Obama and Biden trying to reach a modus vivendi with the Islamic Republic. Sanctions relief, diplomacy, outreach, engagement. Yet a stable accommodation remained elusive. Why? Veterans of the Obama team and their ideological supporters blame America’s supposed “war mongers.” The claim is absurd. The central argument of the hawks has never been that conflict is desirable. It is that the Islamic Revolutionary Guard Corps is a revolutionary jihadi organization that can only be deterred, not seduced. Most Americans would prefer engagement to work. They are not eager for confrontation. They will give diplomacy every chance to succeed. But they are not willing to ignore reality indefinitely. The problem was never those of us warning about the impossibility of a lasting accommodation with Tehran. The problem was Tehran itself. When engagement fails year after year, American leaders return to deterrence not because American hawks are politically powerful, but because their diagnosis is far more accurate. We hawks did not scuttle engagement. Tehran did.

The traditional concern has been that repeated cycles of weight loss followed by weight regain ("yo-yo dieting") may worsen obesity, increase fat mass, reduce muscle mass, lower metabolic rate, and increase cardiometabolic risk. However, a recent review published in the journal The Lancet Diabetes & Endocrinology challenges this long-held belief. The Clinical Reality Approximately 50% of adults worldwide are actively attempting weight loss. Unfortunately, most weight-loss interventions are followed by varying degrees of weight regain, creating repeated cycles of weight loss and regain over time. The key clinical question is: Is weight cycling itself harmful, or is persistent obesity the real problem? What the Evidence Shows Current human evidence does not support a causal relationship between weight cycling and major clinical harm. Most adverse outcomes previously attributed to weight cycling may actually be explained by: • Ageing-related muscle loss • Reverse causality from underlying illness • Earlier onset and longer duration of obesity • Repeated exposure to obesogenic environments • Greater cumulative burden of obesity itself Understanding the Figure The accompanying figure illustrates that regardless of the intervention used: • Lifestyle modification produces modest weight loss with gradual regain. • Very-low-calorie diets (VLCD) achieve greater initial weight loss but substantial regain after treatment cessation. • Incretin-based therapies achieve the largest weight reduction, but discontinuation is followed by partial weight regain. The message is clear: Weight regain is common after virtually all weight-loss interventions. This does not mean that the initial weight loss was futile. A Paradigm Shift The critical clinical insight is that transient weight loss continues to provide meaningful benefits even when some weight is regained later. Documented benefits include: • Improved glycemic control • Reduced hepatic steatosis • Lower blood pressure • Improved lipid profile • Better physical function • Enhanced quality of life • Reduced cardiovascular risk Implications for GLP-1RA and Dual Agonist Era With increasing use of GLP-1 receptor agonists and dual incretin therapies, clinicians frequently encounter weight regain after treatment discontinuation. The new evidence suggests: Weight regain after stopping therapy should not be interpreted as proof that treatment failed. The period of reduced adiposity may still confer important metabolic and cardiovascular benefits. Sarcopenic Obesity Perspective Theoretical concerns suggest repeated weight cycling may preferentially restore fat mass rather than lean mass. However, robust human evidence demonstrating clinically meaningful sarcopenia attributable solely to weight cycling remains limited. The greater threat appears to be: Persistent obesity combined with ageing-related muscle loss rather than weight cycling itself.
