βοΈ Sleeve gastrectomy (SG) is the most performed bariatric procedure worldwide, but long-term issues such as suboptimal weight loss, recurrent weight gain, and de novo GORD are common compared with Roux-en-Y gastric bypass (RYGB).
π Long-term RCTs show that up to 33% of SG patients achieve <20% total weight loss, with de novo GORD in ~40β50% and higher rates of oesophagitis than after RYGB.
π§ An IFSO international Delphi consensus (2025) established standardized definitions for suboptimal response and relapse, emphasizing composite outcomes (weight loss metabolic control).
π Obesity management medications (notably liraglutide) are recommended before revisional surgery, within a multidisciplinary care model.
π For refractory GORD or poor outcomes, conversion to RYGB is the preferred option, while other conversions may yield greater weight loss at the cost of higher nutritional and surgical risk, underscoring the need for individualized decision-making.
