Short-term outcomes in robotic-assisted versus conventional laparoscopic surgery for rectal cancer: population-based study ➡️doi.org/10.1093/bjsopen/zrag… This population-based study compared short-term outcomes of robotic-assisted versus conventional laparoscopic surgery for rectal cancer using data from the Swedish Colorectal Cancer Registry (2014–2021). Robotic-assisted surgery was associated with a lower risk of conversion to open surgery but a lower rate of complete total mesorectal excision specimens, with no difference in circumferential resection margin positivity or postoperative complications. No short-term oncological advantage was demonstrated between the two techniques, and long-term outcomes are needed. 👏👏👏Carl Mertens , Pamela Buchwald , Peter Matthiessen , Henrik Jutesten , Soran Gadan , Fredrik Jörgren #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery #colorectalsurgery #colorectalcancer

Short- and long-term dynamics of gut microbiota, highlighting Fusobacterium nucleatum, Parvimonas micra, and Peptostreptococcus stomatis after colorectal cancer resection: prospective cohort study ➡️doi.org/10.1093/bjsopen/zrag… Curative colorectal cancer resection caused rapid perioperative gut microbiota changes, including transient reductions in diversity. Colorectal cancer-associated bacteria (Fusobacterium nucleatum, Parvimonas micra, and Peptostreptococcus stomatis) were markedly decreased after surgery and remained suppressed for at least 6 months. These findings suggest that tumour-associated bacteria depend on the tumour microenvironment and may serve as stool-based biomarkers for monitoring recovery and recurrence risk. 👏👏👏Tomosuke Mukoyama , Kimihiro Yamashita , Masafumi Saito , Mitsugu Fujita , Seiichi Omura , Takuo Emoto , Tomoya Yamashita , Masayuki Ando , Kyosuke Agawa , Kota Yamada , Akihiro Watanabe , Tomoki Abe , Takao Tsuneki , Yukari Adachi , Ryuichiro Sawada , Yasufumi Koterazawa , Hitoshi Harada , Naoki Urakawa , Hironobu Goto , Hiroshi Hasegawa , Shingo Kanaji , Takeru Matsuda , Taro Oshikiri , Yoshihiro Kakeji #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery

Liver mobilization during Kasai portoenterostomy: retrospective multicentre analysis ➡️doi.org/10.1093/bjsopen/zrag… This multicentre retrospective study compared outcomes of Kasai portoenterostomy performed with versus without liver mobilization (LM) across major European and North American centres. LM was associated with greater intraoperative haemodynamic instability and transient postoperative hepatic stress, but significantly improved long-term native liver survival without adversely affecting later liver transplantation. These findings suggest that, despite short-term physiological burden, LM may confer a meaningful long-term advantage in biliary atresia. 👏👏👏Marie Uecker , Maximilian Holweg , Maria Hukkinen , Lucas Moratilla-Lapeña , Kristine Dräger , Alexander Domasch , Katja Nickel , Sören Wiesner , Cornelius Jakob van Beekum , Jörg Fuchs , Uta Herden , Francisco Hernández , Mikko Pakarinen , Riccardo Superina , Joachim F Kuebler , Christoph Slavetinsky , Omid Madadi-Sanjani #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery

Perioperative faecal microbiome dynamics in patients undergoing rectal cancer surgery in the IMARI-trial ➡️doi.org/10.1093/bjsopen/zrag… In a multicentre prospective cohort, the authors longitudinally profiled the faecal microbiome before and on postoperative day 4 after rectal cancer surgery, stratified by selective digestive decontamination and ileostomy status. Surgery was the dominant driver of change, with uniform postoperative loss of alpha diversity and rises in facultative anaerobes (for example Enterococcus, Klebsiella); ileostomy and SDD contributed smaller, selective effects. These data define early perioperative disruption and provide a foundation for linking microbial trajectories to clinical outcomes and for evaluating microbiome-informed perioperative strategies. 👏👏👏Kiedo Wienholts , Anne E Petersen , Claire P M van Helsdingen , Kevin Talboom , Mark Davids , Johannes H W de Wilt , Wouter J de Jonge , Pieter J Tanis , Roel Hompes , Joep P M Derikx on behalf of the , IMARI-study group #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery

First series of da Vinci single-port robotic-assisted cervical oesophagectomy: single-centre IDEAL stage 2a/2b study ➡️doi.org/10.1093/bjsopen/zrag… This study presents the first clinical experience with single-port robotic-assisted cervical oesophagectomy (SP RACE) using the da Vinci SP system in 20 consecutive patients with oesophageal cancer. The procedure was technically feasible but was associated with substantial perioperative morbidity in a highly selected, co-morbid patient population. These early results underline the need for careful patient selection and further prospective studies to define the role and safety profile of SP RACE. 👏👏👏Vladimir J Lozanovski , Luca Bellaio , Edin Hadzijusufovic , Olga Adamenko Meier , Franziska Renger , Christoph Wandhoefer , Suzanne S Gisbertz , Richard van Hillegersberg , Hauke Lang , Peter P Grimminger #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery #some4UGI #uppergi #esophagealcancer #gastriccancer @ISDE_net @Augishealth @roux_group @T4UGIS #ESDE @Young_ESDE

Preoperative carbohydrate antigen 19-9 as a prognostic biomarker in colorectal cancer with synchronous peritoneal metastasis: multicentre cohort study ➡️doi.org/10.1093/bjsopen/zrag… Overall survival curves based on preoperative serum levels of tumour markers in patients with colorectal cancer and peritoneal metastasis in Cohort 1 were determined. Preoperative serum carbohydrate antigen 19-9 (CA19-9) and carcinoembryonic antigen (CEA) levels were associated with overall survival. However, CA19-9 levels exhibited a stronger, concentration-dependent association with prognosis compared with CEA levels. 👏👏👏Keita Tashiro , Yoshiki Kajiwara , Hideki Ueno , Hirotoshi Kobayashi , Kenjiro Kotake , Kenichi Sugihara , Yoichi Ajioka #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery #colorectalsurgery #colorectalcancer @FightCRC #StepUp4CRC

Effect of noise isolation during general anaesthesia on the incidence of moderate-to-severe pain after major abdominal surgery: multicentre randomized clinical study ➡️doi.org/10.1093/bjsopen/zrag… This multicentre randomized clinical study in 304 adults investigated the effect of noise isolation during surgery on the incidence of moderate-to-severe pain. In the 24-hour period after surgery, the incidence of moderate-to-severe pain was significantly lower in the noise isolation than control group (23 versus 49%). Intraoperative noise isolation was found to be an effective, safe, and non-invasive preventive intervention that significantly lowered the incidence of moderate-to-severe pain after major abdominal surgery. 👏👏👏Fang Xu , Dong Chen , Cong Wang , Yi Yang , Yingcai Wu , Shuai You , Guangyou Duan #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery

Anastomotic leak after colorectal surgery: current landscape and future directions ➡️ doi.org/10.1093/bjs/znag041 🔗 Anastomotic leak remains one of the most feared complications after colorectal surgery, with major consequences including sepsis, reoperation, permanent stoma, and poorer oncological outcomes 📊 Leak rates remain substantial despite modern care: 3–6% after ileocolic anastomoses and up to 10–20% after low colorectal anastomoses 🧬 Anastomotic healing is multifactorial, involving surgical technique, perfusion, host immune response, and the gut microbiome 💡 ICG fluorescence angiography may improve perfusion assessment, particularly in rectal cancer surgery, but standardized quantitative thresholds are still needed 🦠 The microbiome is emerging as a key contributor, with collagenase-producing bacteria potentially weakening the anastomosis 🎯 Future prevention will likely require personalized risk stratification and targeted interventions rather than a one-size-fits-all approach Work by Jack A Helliwell , Aaron Quyn , David G Jayne #SoMe4Surgery #MedTwitter #SurgEd #Surgery @RCPSGTrainees @aecirujanos @SEIQuirurgica @iss_sic #MedicalTechniques @BJSAcademy @young_bjs @BJSOpen @evanscolorectal @robhinchliffe1 @bplwijn @MalinASund @nfmkok @TejedorPat @paulo_sutt @PVaughanShaw @JJEarnshaw @juliomayol @ksoreide #colorectalsurgery #StepUp4CRC @FightCRC @ACPGBI #ERAS @dice_europe #Crohn #proctology @Dukes_Club @ACPGBI_EduTrain @AECP_FAECP @PelvExGroup @escp_tweets @YouESCP #TeachMeColoproctology #Some4COLoprocto

Interhospital variations in practice and technical outcomes for endoscopic resection of early oesophagogastric adenocarcinoma: multicentre CONGRESS data set analysis ➡️doi.org/10.1093/bjsopen/zrag… Using the CONGRESS multicentre registry (2015–22), this study evaluated interhospital variation in outcomes following endoscopic resection (ER) for T1 N0 oesophagogastric adenocarcinoma. Among 1215 patients from 28 centres, high-volume centres demonstrated significantly lower rates of R1 resection, procedural complications, and progression to surgery compared with low-volume centres, even after adjustment for case mix. These findings indicate substantial practice variation and suggest a volume-outcome relationship in ER for early OG cancer. 👏👏👏Kirsty Cole , @gastro3570 ,@jamesgossage9 , Natalie Blencowe , @cswathikan , Tom Crosby , Neil M Davies , Richard P T Evans , @EwenGriffiths , @Sivesh93 , @MarkarSheraz , @NJEMCP , @TimTheSurgeon , @philip_pucher on behalf of , the CONGRESS Collaborative #SoMe4Surgery #MedTwitter #SurgEd #Surgery @BJSAcademy @BJSurgery #some4UGI #uppergi #esophagealcancer #gastriccancer @ISDE_net @Augishealth @roux_group @T4UGIS #ESDE @young_esde

Development of a standard definition of ‘no-option’ and ‘poor-option’ for revascularization in chronic limb-threatening ischemia ➡️doi.org/10.1093/bjs/znag040 🦵 How should we define “no-option” or “poor-option” CLTI? 🌍 This modified Delphi study brought together 164 vascular specialists from 30 countries to develop an expert-validated definition. 📊 Consensus supported a multidomain ABRFC framework: arterial anatomy, biology, risk, function, and context. ⚠️ “No-option” CLTI includes desert foot, prohibitive medical risk, non-functional limb, or patient refusal of revascularization. 🔍 “Poor-option” CLTI reflects combined factors such as severe infection, lack of autologous vein, or treatment non-compliance. 🎯 This framework can standardize clinical assessment, trial design, reporting, and future guideline development.
