'Talking about dying won’t make it more likely. Death rates remain a stubborn 100% despite medical advances, a rate that looks unlikely to change'. Fab piece by @drkathrynmannix on new study of (over) investigation of pts who die <6mths later #Last1000Days theguardian.com/commentisfre…

Dr Ollie Minton on shared decision making in emergency cancer presentations. 2/3 of these patients will die within a year. Plan for the best, prepare for the worse - gives patients the best of everything in a pragmatic and patient centred way. @OncologyForum

Patterns of acute hospital & specialist palliative care (SPC) use among people with non-curative upper gastrointestinal cancer showed less likelihood of hospital re-admissions if patients were referred to the hospital SPC team⁦ @DrFlissMurtaghlink.springer.com/article/10…

Crazy. If only hospices had proper investment and greater number of beds. Makes asking someone about their preferred place of dying a slightly pointless exercise.
Another important and still very much live issue I wrote up for @thetimes Hospices face turning dying patients away as costs rise without NHS support. FREE to read today thetimes.com/uk/healthcare/a…

New ASCO Guideline on Palliative Care for Patients with Cancer ascopubs.org/doi/full/10.120… 1. Early referral to SPECIALIZED PC leads to improved QOL, mood, higher likelihood of documenting EOL preferences 2. PC teams are fundamentally interdisciplinary 3. PC benefits caregivers emotionally/psychologically 4. Referral should be "timely" - do not wait until cessation of therapy 5. PC helps pts with hematologic malignancies and those on phase I trials

For the people in the back. @ASCOPres says during presidential address that palliative and supportive care must be woven into all of cancer care. A first. Palliative care on the main stage. #hapc

About how other clinicians refer (or not) to palliative care... The genius of @DrJRubenstein, animating a conversation of desperation we held a few weeks ago. I'm very proud of this one. 🔥if this sounds familiar ✋🏽if you plan to take this advice youtu.be/nkWrJeBq0hI?si=ZW7a…

I've written a piece on the relationship between end of life and the future of the NHS. Part of the #NHS2048 @reformscotland series on how the NHS can be sustained and improved.
“the NHS, like most western healthcare systems which were designed to treat and fix, retains an ambivalent relationship with incurable disease and dying” @Palliative_Scot writes for #NHS2048 ⬇️ reformscotland.com/nhs2048/h…

Great paper from @UHSussex @drol007 and team - ⬆⬆ unplanned ED use in last 90 days of life regardless of condition; 10 primary care contacts in last yr ⬇ likelihood of death in 🏥; palliative care registers don’t impact on dying in 🏥 but do ⬇ unplanned emergency admissions

Lovely to chat to @DrJRubenstein this week. Here's his breakthrough short video, ICYMI. How many times have you avoided a necessary palliative care conversation 'in case it upsets people'? youtu.be/BbNi_-wYXJE?si=ZzE9…
