@acutemedicine people. We’re hearing of increases in AMU bypass due to boarding. So patients who are frail and vulnerable are day 8 on AMU whilst those we can discharge within 72 hours are going direct to specialty. This gamification is really concerning.

Twenty years ago we had a geriatric day hospital where we could see patients and give transfusions. Managers closed it. Now the new thing is SDEC where we can now give transfusions and see patients. Managers love it.

Elderly/frail patients should not be excluded from SDEC. Can interfacing with frailty teams be an idea? Rather than lying around in the corridor of the ED? Being an inpatient carries real harms, often avoidable.

Some might remember my proposed education initiative to improve feedback and the general education experience for doctors in acute medicine last year. Well, I have now been running the ‘feedback clinic’ for a year, and I wanted to share my learning/experiences
In spirit of sharing ideas: I’m piloting a new education initiative in Aug for trainees in acute med. I hope to improve clinical feedback for docs & give more opportunities for them to get the WPBAs they need for portfolios by removing their need to ‘hustle’ to get them completed
