There’s difference between high-volume center and high-performing center. Poor performing high volume centers exist. Opposite also true. We can debate in good faith. What we can’t debate is that we can’t execute trials in the US anywhere nearly as well as @MarcBesselink and team.

Never any harm in a second opinion. Patients with potentially resectable tumors should push their surgeon on their experience and outcomes (morbidity rate, leak rate, mortality rate). As surgeons, we should know this information in detail and be able to comfortably share it.

A randomized trial finds no advantage to using abdominal drains after left pancreatectomy. In fact, patients without drains had fewer complications and shorter stays.

What does new onset diabetes mean in surgery for pancreatic cancer?
✅It’s unique to PDAC - it’s rare in other neoplastic HPB processes like ampullary or distal CCA.
🧐Its under diagnosed - it must be screened for or it won’t be identified.
🤢It increases the risk of DGE.
😁It reduces the risk of POPF.
Do you routinely screen for endocrine dysfunction in your whipples?
Do you use it in scores to predict who will develop DGE and POPF?
