Great point by our SW England Lead: Frequent Callers are vulnerable & complex with needs that are not being met with various reasons. Care needs to be individualised and person-centred. The SCR AI will allow you to see what’s been done to date. Consider what support is needed.
Summary Care Record Additional Information (SCR-AI) increases the amount of info to support clinical decisions. The SCR-AI should help understand what’s already in place to support #FrequentCaller patients get the @swasftRightCare/@nhsrightcare for this vulnerable group @FreCaNN

Great to see tools such as this being used to tailor care to the individual. Ensure your care is person-centres and we can support patients to reduce their reliance on 999 and improve self-resilience.
Great for identifying unavailable services that would have helped those calling 999 frequently and need better/different/more support.

Good pain management is key to providing the right care sooner and reduce the likelihood of the problem getting worse and therefore the need for further healthcare services and any potential wait. Saving our 999 services for those that need live saving treatment.
