Medical doctors use the "doctor" title as a courtesy. Actually they initially adopted the title to gain legitimacy & elevate the field.
The title accurately belongs to academic doctorate holders.

If we had a higher GDP per capita, there will be no need for prosperity teaching. I say this as someone who has attended church in US mid-affluent neighborhoods.
It’s purely demand and environmental issue. People will still go to church but serial preaching focus on complete surrender to Christ and character development.
